During the course of the investigation, it was determined that operation failed to comply with minimum standards by providing falsified CPR documentation for a caregiver present during an incident.

A caregiver in the pre-k classroom, when asked how many children they had stated there were 13 children present. When the CCR inspector counted children there were 14 children present. Furthermore, A caregiver in the school age classroom, when asked how many children they had stated there were 17 children present. When the CCR inspector counted children there were 16 children present.

The 2 year old classroom had 12 children with one caregiver. This was corrected at inspection when another caregiver entered to assist the other caregiver bringing the operation into compliance.

3 infants were observed not buckled into the kidney bean toddler table during meal time. THis was corrected at inspection when the infants were removed from the table.

It was determined through the course of the investigation that there was a period of 58 seconds where no one was responsible for a 4 yr old child that was left on the playground during transition.

Toilet bowl cleaner was observed in an unlocked cabinet that was accessible to children in care. This was corrected at inspection when the cleaner was removed from the cabinet and put out of reach of children.

It was determined through reviewing video footage of classrooms, that a caregiver acted in an aggressive manner toward children in care. The caregiver can be seen ushering children to another area of the classroom by aggressive force, pulling of arms and pushing children in care.

A caregiver in the two year old classroom was observed supervising 12 two year old children alone. This was corrected at inspection when the caregiver moved a child to another room bringing the classroom into ratio.

A caregiver served a child whole milk which was a known food allergy documented in a signed note from the child's physician.

A child in care with an allergy to whole milk, as documented in a signed note from the child's physician, did not have an emergency allergy plan on file.

A can of sunscreen and a bottle of handsanitizer were observed within reach of children in care. This was corrected when the containers were put put up out of reach during this inspection.
