A child file reviewed for a child in care did not have any immunizations on file available for review.

The operation has not provided the Parent's Rights information to parents as required to be included in the operational policies.

The annual training year evaluated for the primary caregiver of 3/21/2024 - 3/21/2025 did not have any of the required health topics.

Two dogs at the operation are not up to date on their rabies vaccinations. One dog needed an updated vaccine in March of 2024 and the other dog needed an updated vaccine in April of 2024.

The caregiver has a current certificate in CPR but the curriculum did not include hands on practice with a CPR manikin.

One infant in care that is under 12 months of age was observed sleeping in a play yard with a blanket.

Sign in and out logs were not available for review for the month of March 2023.

The provider was aware of an individual with a background check listed under the operation that is not a household member that has/had an open Child Protective Services case and did not report it in a timely manner.

The fire drills, smoke detector, and carbon monoxide tests were last practiced and documented in September 2022 and these are required to be completed once monthly. Additionally, there last practiced and documented fire drill is also dated for September 2022. The operation did not practice and document four severe weather/shelter in place drills and four lock-down drills as required to do so per the standards for the calendar year of 2022.

The primary caregiver's annual training was not available for review at inspection. The infant feeding instructions for two children that require infant feeding instructions to be updated every 30 days by parents were not available for review.

The primary caregiver does not have current CPR training and First Aid training in file.

The operational policies were not complete as they are missing information to parents about safe sleep for infants and did not have information about the indoor and outdoor physical activity as required. The operational policies were also missing information regarding the most recent Child Care Regulation Inspection report (previously known as Licensing) and how to access the HHSC website (link provided) and how to contact local CCR (Licensing).

The operational policies did not include information for promotion of physical activities to parents.

The last fire drill documented was dated for 10/2020.

The children's product certification form was last completed in 2020. **This was corrected at inspection when the provider re-signed the form at inspection.

The primary caregiver only had 1 hour of annual training available for review.

Two children's files reviewed were missing the parent signature on last page of the child admission form. One of these two children was also missing the following information: phone number for person listed for release by the parent individuals and the emergency contact address was missing for emergency person listed by parents. **The two child's admission informations was corrected when the parent signature at inspection as well as the emergency address and phone number for person of release listed for the child whom was missing it.

One pet vaccination was unable to be reviewed upon request at inspection.

A child's file did not have proof of safe sleep for infants that are 12 months and younger in file when reviewed.

One child did not have up-to-date feeding instructions in file for review.
