It was determined a caregiver at the operation used prohibited punishment when the caregiver swatted a child on the hand.

Throughout the course of the HHS Investigation, a caregiver failed to keep her hand on a child in care while the child was on the elevated diaper changing station. This resulted in the child in care falling to the ground.

It was determined throughout the course of the HHS Investigation the caregiver failed to maintain the level of supervision required for a young child in care while on the playground. Surveillance footage revealed the child was out of the caregiver?s view for a total period of 6 minutes. Within those 6 minutes, the child was left alone outside unsupervised on the enclosed playground for 3 minutes and 15 seconds.

It was determined throughout the course of the HHS Investigation the operation failed to report to CCR within the required time frame. The incident occurred on 1/31/25. The incident was not reported to CCR until 2/3/25.

It was determined throughout the course of the HHS Investigation that a caregiver withheld food from a child in care when the child refused to put their hands in their lap. The caregiver also threatened to remove other children's plates if the children didn't stop yelling.

While reviewing surveillance footage for the HHS Investigation, infants in care were observed in restrictive devices for 36 minutes, 41 minutes, 26 minutes, 25 minutes, 47 minutes, and 26 minutes.

Throughout the course of the HHS Investigation, a 4-month-old infant in care was observed with a blanket in the crib while reviewing surveillance footage.

Two infants in care were observed sleeping in a restrictive device while reviewing surveillance footage for the HHS Investigation.

During the course of the investigation it was determined that a caregiver used profane language in front of children in care.

Video footage was observed, showing a caregiver snatching a toy away from a child and stated ?You are being ridiculous, and I don?t like it.? It was also determined that the same caregiver treats children differently based on their preference of child.

It was determined throughout the course of the DFPS Investigation that a caregiver did not hold or comfort an ill child in care.

It was determined throughout the course of the DFPS Investigation that a caregiver grabbed a child in care and picked the child up where their feet weren't touching. It was also determined that the caregiver was rough with a child in care on multiple occasions. The caregiver spoke to children in care in a harsh tone by stating "You are getting on my nerves."
