Primary caregiver's First Aid/CPR expires on 11/30/25.

The CPSC form was not signed. This was corrected when the form was signed.

Three animals do not have current vaccinations. They expired on 09/24.

The provider has not certified her CPSC form from HHSC for the year.

The emergency preparedness plan does not address how children under 24 months of age, who have limited mobility , or who otherwise may need assistance in an emergency, such as children who have mental, visual or hearing impairments.

The assistant caregiver did not wash a toddler's hands after their diaper was changed. The child went back to playing. Childcare Regulation had to remind the assistant caregiver to wash the child's hands.

The primary care giver's 12 year old child was upstairs by themselves.

One of two personnel records evaluated was missing the required number of annual training hours (short 5 hours).

Two of four children's records evaluated did not have addresses for the emergency contacts.

One of four children's records evaluated did not have a health statement as required after the parent signed off the child had been examined in the past year and would provide one within the year. The health statement should have been provided by 10/25/22.

Follow up on a DSHS immunization audit of the operation, shows that one of two corrections needed have not been corrected.

The provider has not notified parents that liability insurance is not carried which was supposed to have been done by 5/25/21 if it was not obtained.

The operational policies do not include the promotion of indoor and outdoor physical activity section requirement that was added to minimum standards last year.
