This standard was found deficient as 2 employee files did not have a notarized affidavit in their file.

I observed a caregiver at the operation that does not have background check submitted to CLASS.

I observed that the storage area for the children was not clearly labled for each child's belongings.

While completing the health and safety audit, I observed that the operation did not have a section addressing shelter in place.

I observed an infant during naptime in a dark room that did not have adequate lighting. The door was shut and the infant was alone.

As I was completing the walkthrough of the operation, I was told by the director and her husband that their son was living in the home and they showed me his bedroom. I reviewed CLASS, and their son is not currently documented as having a completed background check. He was not present at the time of the inspection and the director stated they would run the checks that day.

While completing the health and safety audit, the operation was unable to provide me with personal records for the directors husband, who lives in the home.

I observed that cabinets in the kitchen and bathroom contained cleaning supplies that were accessible to children. I observed that there were no locks on the cabinet doors.

I observed a storage container in the outdoor play area that was accessible to children and did not have a hinged lid.

The provider did not have training in the following topics: 1. Preventing and responding to emergencies due to food or an allergic reaction 2. Administering medication 3. Preventing and controlling the spread of communicable disease, including immunization 4. Handling, storing, and disposing of hazardous materials including compliance with 747.3425

The provider did not have complete food allergy for a child that is enrolled.

Two children files did not have verification that the parent received a copy of the operational policies.

The provider did not have a current notarized employment affidavit.
