Two caregivers did not have the required 24 hours of annual training.

A child was left in a classroom unsupervised.

Operation failed to notify Licensing of a reportable illness within the timeframe.

A standing electric fan was on the floor in the toddler room. A child was sticking their finger though the fan screen and the broken middle area.

Caregiver in the toddler room did not wash children's hands after the child used the toilet.

Two children do not have current immunizations in their file.

An electrical outlet low to the floor and accessible to children, was uncovered in the indoor play area. A toddler was playing near the outlet. This standard was corrected at inspection when the staff immediately covered the outlet.

Four infant feeding instructions for infants not yet on table food, were not updated at the 30 day mark. They were last updated on 4/3/22, 4/5/22, 1/5/22, 4/5/22.

Caregivers in the toddler class did not wash hands after diapering one child and diapering a second child. Caregivers wore gloves and changed gloves between diapering but did not wash hands between diaper changes.

Two children aged 4 years 8 months and 4 years 10 months do not have vision and hearing screening records in their files.

Two caregivers wearing gloves during diaper changing did not wash their hands after and between diaper changes.

Three classrooms of children under four years of age, had unlabeled bleach water spray bottles in an unlocked cabinet, accessible to children. This was corrected at inspection when caregivers placed the spray bottle out of reach of children and labeled it.

This standard was reevaluated and is still out of compliance. Some children signed in were missing time in and other information. This was corrected at inspection when caregivers filled in the missing information.

The caregivers did not have the child tracking sheet in the classroom and did not carry the child tracking sheet thorough out the day to ensure all children are accounted for.

A child was left in the classroom alone for an unknown length of time.

The child tracking sheet for the day of incident is incomplete. Seven children signed in and nine children signed out. Some children do not have sign in/out times or parent initials.

One caregiver working directly with infants does not have current training in SIDS, shaken baby syndrome, and early childhood brain development.

One caregiver evaluated had only 16 hours of the required 24 hours of annual training for the previous year.

Infant feeding instructions are not dated and are updated only and when the infant feeding changes. They are not updated every 30 days.
