A child was accidentally left unsupervised in a classroom for a brief 1 minute and 29 seconds before the caregiver realized that the child was not with the rest of the group. The incident occurred during a transition period.

Three caregivers who work with children 24 months and youinger did not have annual training in SIDS, Shaken Baby, and Early Brain Development.

The operation did not have a carbon monoxide system installed. This was corrected at inspection with the purchase and installation of a plug in carbon monoxide system.

The operation did not have an annual health inspection.

The caregiver left a child unsupervised in the indoor gym. The investigation violation was marked corrected at inspection, as the employee was no longer present or employed.

A caregiver left a child unsupervised in the gym for approximately 30 seconds before the child's parent found the child unsupervised. The caregiver had taken the children outside to the playground, and did not realize that the child was missing from the group.
