A caregiver did not wash a young child's hands with soap and running water after a diapering. When CCR brought it to the caregivers attention, the deficiency was corrected by redirecting the child to the hand washing sink to assist the child with washing their hands with soap and running water.

Based on information obtained during a DFPS investigation it was determined that a caregivers could adequately supervise children and immediately intervene when needed children while sitting down during outdoor at activities pose a risk to children.

Based on a photo, a video, and interviews with collaterals, it was determined that a caregiver transition a group of children into the gym with glass on the floor that was not completely cleaned up which resulted in a child obtaining a small piece of glass in their knee.

A caregiver was in charge of a group of young children without an initial background check on file. The deficiency was corrected during the inspection when the caregiver was removed from the classroom.
