During the inspection, an outdoor play structure had exposed and rusting metal surfaces, a bench and the fence had exposed splinters and nails, several tricycles were missing parts and had sharp plastic edges, and the soccer goal had sharp rusty edges where it had broken.

During the inspection, the operation did not have a fire inspection of file since 2023.

Video evidence and witness statements confirmed the violation.

When a caregiver brought a child to another caregiver in the lobby area of the operation at the beginning of the day, the caregiver did not notify that caregiver that they were handing over the care of the child to that caregiver. As a result, the child left the lobby area and was discovered unsupervised in a classroom; the child was unsupervised for approximately 4 minutes.

This standard was tasked over for the investigation and was found to be deficient. The operation did not provide an incident report to the parent within the required time frame.
