At the time of the inspection two children were competing over a popular item and there was only one item available.

At arrival the director was present in a classroom providing ratio support and was unable to immediately perform required director duties, including access to and review of required records. The licensing representative arrived and due to staffing needs, the director remained in the classroom until approximately an hour until another staff member arrived to relieve her. As a result, licensing waited approximately one hour before the director was available to retrieve and present records for review. It was determined that the operation is having ongoing staffing challenges that have limited the director's ability to consistently perform director responsibilities as required.

There was no documentation of the annual health sanitation inspection to review at the time of the inspection.

There was no documentation of a fire inspection to review at the time of the inspection.

The operation opens at 06:30 AM and the first meal was not served or offered until 10:00 AM.

A qualified caregiver was not left in charge of the operation at the start of the inspection.

According to the timesheet the current director is not at the operation 75 percent of the time.

The operation has not conducted a fire emergency drill since December 2021.

A caregiver at the operation served an identified food to a child that was allergic.

A caregiver at the operation did not review food allergies before serving a restricted food item to a child in care.

A caregiver did not use good judgement when serving a child in an unfamiliar classroom an allergic food item. The caregiver did not review food allergies for the children in the classroom.

A garbage can with garbage was observed on the back porch without a closed lid and was accessible to children. There was also used cups and tissues observed in the grass areas and along the fence line of the playground. This was corrected during the inspection when all garbage was made inaccessible to children.

The three year old classroom did not have an age appropriate activity plan posted. This was corrected during the inspection when the correct activity plan was posted.

Three caregivers were missing the required 24 training hours for their last training year. Training certificates were not available for review during the inspection.

Ten admission records were evaluated and three records were missing health care provider statements signed by a physician.

Ten admission records were evaluated and the following information was missing. - Two records were missing the name and phone number of the school that the child attends away from the center. - Four records were missing the name, address, and telephone number of the child's physician or an emergency care facility. - Five records were missing information for the emergency contact person such as the name, address or phone number.

Four tricycles observed on the trike track outside were missing a pedal. The tricycles were removed from the play area during the inspection.

Ten admission records were evaluated and three records were missing immunization records.
