2 caregivers did not know how many children they had.

During any investigation there was evidence to support the allegation that a caregiver would yell at the children for discipline

The operation did not report a suspected abuse within the required 48 hours.

During an investigation it was found that a caregiver grabbed a child by their arm for discipline.

During an investigation, it was found that 2 children had to share a seat on the bus during after school pick up on at least one occasion. The operation has changed their transportation procedure in response to this incident.

One child was missing one immunization.

A fire inspection had not been completed for the operation since 2023. A sprinkler system test was completed, but after calling the city, it was determined that the operation had not been inspected since 2023.

One child did not have a physician statement indicating the child was well enough to attend the child care operation.

The operation did not have a current sanitation inspection.

There was a caregiver present caring for children who did not have a cleared background check.

During an investigation, it was found that a caregiver kicked a chair into the table with a child sitting in the chair. It was also observed on video a caregiver grabbing a child's arm aggressively, grabbing a child by the jaw and sitting a child down firmly on the floor. A caregiver was also seen yanking a toy out of a childs hands.

There was no documentation of a fire drill for February 2023. This was corrected when the drill was completed.

During the inspection the infant feeding written instructions did not have a parent signature for infants not on table food.

During the inspection children were observed transitioning from free play to the table for lunch without washing hands prior to food being placed in front of them. This was corrected by the children washing their hands.
