There was no current Health inspection Report to review during the inspection. Health Inspection report was dated 4/19/2023.

A review of the Employees Who Left Employment in 2024 data revealed that the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.

The mulch in the active play use zones was not six inches in either playground.

The fire inspection had not been updated for 2024.

The loose fill on the Pre-K and School Age Playgrounds was less than six inches in the use zones.

Two of ten children did not have updated immunizations in their records.

Three infants under 11 months were observed sleeping in restrictive devices for approximately 5-10 minutes. This was corrected when the staff in charge asked them to remove them.

Two 18 month old children were observed watching t.v. This was corrected when the staff member in charge asked them to turn it off.

There was sufficient evidence to support the allegation that a child was inappropriately punished by having her face grabbed and pulled by a caregiver.

The turf on the playground for the infants is coming up and is a tripping hazard to the children.

The operation does not have a current annual fire inspection as the last was conducted on November 11, 2020.

A caregiver who did not have a cleared background check was present at the operation and had access to children in care.

The diaper changing mats in the infant, toddler, and one's classrooms were torn and/or discolored.

In the one's classroom, multiple sippy cups and bottles were not labeled with the children's names. Note: Corrected when caregiver labeled each bottle and cup.

Mulch on the school age playground was very low as it was missing in multiple areas and grass was growing through.

Three caregivers who did not have cleared background checks were present at the operation and had access to children in care.

A child was blocking the entrance door to the classroom during naptime. This was corrected when the child was moved fromt he entrance door and placed in another area of the classroom.

The was a 14 month old child in a classroom with a 46 month old child.

There was one cargiver with 5 infants aged 0-11 months old in the infant classroom

Two of the 5 employee files did not have the rrequired 24 hours of required annual training hours.
