Operation was over ratio by one child. One caregiver was providing care for one chilld under 17mo, four children 18mo-4yo, and seven children 5-12yo.

Based on the information obtained during the DFPS investigation, it was determined that a caregiver tried to force a child to sleep by roughly handling the child then grabbing the child?s arm and holding on to prevent the child from moving off the nap cot.

Two out four children's records reviewed are missing immunization records.

Four staff files were reviewed. One file is missing 16 hours of pre-service training. Three files are missing annual training hours, including the following topics: Abuse/Neglect, Hazardous Materials, Food Allergies, Administering Medications, Emergency Preparedness and Building Safety.

There were a total of 13 children present at the operation. A caregiver counted in ratio stepped out of the room and into the kitchen to prepare supper.

Operation became out of ratio when six school-age children arrived at the operation. There were a total of 13 children present at the operation, caregiver in the toddler room was providing care for seven children ages 4mo-3yo. The age difference between the youngest and oldest child in the room exceed 18 months.

The fire extinguisher in the transportation vehicle was not currently serviced. The service date on the tag was September 2022.

The activity plans posted in the toddler room and afterschool room were not current. The dates documented on the plans were 12/4-8/2023 and 12/11-15/2023. Note: This was CORRECTED during the inspection as current lesson plans were obtained and posted in each room.

The operational policies reviewed were missing the following information: The Procedures for supporting inclusive services to children with special care needs. The policy must address the requirements outlined in ?746.2202 of this chapter (relating to What are my responsibilities when planning activities for a child in care with special care needs?).

The pool noodles covering the poles of the shade structure in the outdoor playground were not maintained. The pool noodle material was deteriorating and missing pieces. This posed a safety hazard for children as the pool noodles were accessible to children and smaller children may accidentally ingest small pieces.
