There were three expired medications observed during inspection.

A trashcan containing a soiled diaper, without a lid, and accessible to children was observed during the inspection. This was corrected at inspection when a lid was added to the trashcan.

A child had food allergy plan with appropriate signatures however, did not have the medication as required per the food allergy plan. Without this medication, the plan could not be implemented.

Medication listed on a child's food allergy emergency plan was not in its original box labeled with the child's name.

A cleaning supply spray can was observed on the back of the toilet in a bathroom used by children. The cleaning supply spray can was accessible to children. This was corrected at inspection when the caregiver moved the cleaning supply spray can.

There was sufficient evidence found during the investigation to show that the operation did not immediately notify the parent of a child when a volatile parent yelled and cursed at their child while the child was in care.

It was found during an investigation that a caregiver failed to properly fasten the safety straps on a highchair as the safety straps were fastened, but not tightened, resulting in an infant climbing out of the highchair and falling to the floor, hitting their head.

There was sufficient evidence found during an investigation to support that an infant was able to climb out of a highchair and hit their head on the floor without a caregiver intervening. One caregiver was changing a diaper, and another caregiver was sitting on the floor feeding another infant.

A caregiver did not use good judgement when they did not communicate with a parent if a child needed transportation and left the child behind at the elementary school.

Screen time was observed in a classroom during rest time. This was corrected at inspection when the caregiver turned the screen off.

One caregiver gave the wrong number of children in care when asked twice. This was corrected at inspection when the caregiver counted the number of children and gave the correct number.

Information obtained throughout the investigation shows that a caregiver failed to provide care consistent with a child's special dietary needs when they did not give the child the food and drink needed for their special diet.

One child did not have all of the medication listed on their food allergy plan.

One caregiver did not have CPR training that includes hands-on skill-based instruction.

Information obtained throughout the investigation shows that a child sustained an injury when a caregiver left a backpack with scissors in it accessible to children, resulting in a child cutting another child's finger with the scissors.

There was a caregiver present who was caring for children and who did not have a cleared background check. The caregiver left during the inspection.

During a DFPS investigation, it was found that a caregiver grabbed and pulled a child in care inappropriately.

Two caregivers did not know how old the children were that they were responsible for.

In the act of redirecting a child's behavior a caregiver hit a child with an open hand on their face.

During an investigation inspection caregivers interviewed admitted to the use of time outs. These time outs were explained as placing a child behind the diaper changing station so they cannot see the other children having fun and lasting five minutes or more.
