Out of 4 staff training files reviewed, 2 staff did not have proof of emergency preparedness, communicable diseases, building safety, and hazardous materials training. One staff did not have proof of maltreatment training.

A child was observed being allowed to be in a classroom by themselves during the inspection. This was corrected when a caregiver started supervising the child.

A non-walking infant under the age of 12 months was asleep on a mat on the floor with a blanket during the inspection. This was correctetd when the infant was moved to their crib to sleep in.

Information obtained during an investigation shows that the operation failed to demonstrate good judgement when they brushed ants off a child in the morning but did not check back on the child for ant bite reactions further in the day. The child was picked up by a parent at the end of the day with a swollen foot.

During an investigation it was found that the operation did not notify a child's parent when the child received bug bites while in care, causing the child's food to swell.

There was a bottle of floor cleaner on the floor in the children's bathroom. This was corrected at inspection when the director removed the bottle from the floor and placed it out of reach of children.

Three caregivers were outside and not supervising the children.

Two caregivers were on their cell phones.

Information obtained during a DFPS investigation shows that caregivers failed to use positive methods of discipline and guidance by waiving their finger in children?s faces, pointing in children?s faces, and putting their finger to children?s mouths when being redirected. Caregivers will throw balls towards children when getting their attention.

The director and four caregivers did not have current training in pediatric CPR.

A four year old was playing a video game that did not meet educational goals. This was corrected when the child was picked up.

The operation does not have a current sanitation inspection.

Two infants were sleeping in swings. This was corrected when one infant woke up and one infant was moved to a crib.

The infant classroom had an 8 month old with a 4 year old. This was corrected when the 4 year old was picked up.

One infant fell asleep while drinking a bottle. This was corrected when the bottle was taken away.

The director and four caregivers did not have current training in pediatric first aid.

A walk-through was conducted of the infant classroom. Two infants were observed asleep and unsupervised in the classroom. A caregiver re-entered the classroom after diapering an infant, bringing the facility into compliance.

An infant not yet ready for table food did not have updated written feeding instructions on file for review.

Infants were observed using high chairs that were equipped with safety straps. The safety straps were not fastened. The safety straps were immediately fastened by a caregiver when addressed, bringing the facility into compliance.

A 3 month-old infant was swaddled in the play yard being put to sleep.

The operation does not have a current sanitation inspection.
