Upon arrival the infant teacher was observed in the per-school room seated with the door to the infant room closed. There was no caregiver in the infant room upon inspection.

A 4-year old child wandered away to the restroom without the knowledge or supervision of caregivers.

Several children were observed sitting unfastened in high chairs during meal time. This deficiency was corrected during the inspection as safety straps were immediately fastened following discussion of the deficiency.

A caregiver was observed using a cell phone for several minutes during nap time. This deficiency was corrected during the inspection. The director of the operation and the licensing inspector informed the caregiver that use of cell phones is prohibited while children are in care.

In the infant classroom, the youngest child present was 9 months old, while the oldest child present was 3 years old. This deficiency was corrected during the inspection. The 3 year old was moved to the pre-k classroom.

A caregiver in the pre-k classroom was unaware of the ages of the children in her care and did not have documentation available to reference each child's age.

An daily report was not available for an infant in care.

There were no feeding instructions available for an infant that currently consumes baby formula as regular part of his diet.

Three infants were observed sitting in high chairs without being fastened in with safety straps. This deficiency was corrected during the inspection. Each infant was immediately removed from the high chairs.

Children in the infant and pre-k classroom included a group of which the age difference between the youngest and oldest child exceeded than 18 months. This deficiency was corrected during the inspection. Children were sent home to help meet ratio compliance.

Two caregivers were allowed unsupervised access to children without having a current first-aid certificate.

Children were observed walking and sleeping with a training cup and bottle in hand. This deficiency was corrected during the inspection. The training cup and bottle were both put away.

In the infant classroom, there were 11 children present with 1 caregiver. This deficiency was corrected during the inspection. A substitute caregiver was called to help supervise the classroom.

Infants enrolled at the operation did not have feeding instructions on file to help ensure infants received proper feedings.

Caregivers did not maintain infant daily reports to help assure parents that prompt diaper changes occurred throughout the day.
