Binders taken during transportation route are missing children's information including emergency medical authorization and list of children being transported.

One of the transportation vehicles has 7 non-working seat belts.

Toddler and School-age groups waited for lunch with no planned activities for approximately 30 minutes. Three toddler-age children were in feeding chair (with tray/straps) for that time frame.

Children in the toddler room ages 18mo-2yo were watching Masha and the bear.

Two staff members were absent resulting in the need to combine classrooms and forcing caregivers present to multi-task including meal preparation while there were 34 children present.

One out of six staff files reviewed is missing trainings on Food Allergies, Emergency Prepardness and Transportation.

Multiple mats are torn and while some mats were labeled with names and numbers, they were randomly distributed among the children.

A pre-k age child was placed in a feeding chair (with safety traps / food tray) for lunch. Child was observed sliding through the room within the chair.

Immunization records for two out of the six children's files reviewed were not available at the time of inspection.

Last fire drill was conducted on 03/12/2025 and no carbon monoxide detector checks have been conducted for 2025.

First Aid / CPR training for three out of the six staff files reviewed expired on 07/04/2025.

Rugs and cabinet with tape residue were removed from the school-age room. No toilet brushes / plungers were observed within the children's restroom.Six electrical outlets were observed uncovered: two in the multi-purpose room, three in the toddler room and one in the prek room. This was corrected at inspection with provider covered the outlets. The wall in the multi-purpose room and chipping paint within the rooms have not been fixed. There was a torn garbage bag within the playground and an uncovered sandbox.

Children were not observed participating in the planned activities listed under the lesson plans posted.

Rugs in the school-age room have multiple stains and a cabinet has tape residue. A toilet brush and plunger were observed within the children's restroom. There is chipping paint on the walls of the front / multipurpose room and prek room. Six eletrical outlets were uncovered.

A caregiver was observed changing a child's diaper in the pre-k room and placing the soiled diaper on the floor.

2 of 6 children's records reviewed were missing the child's a statement of the child's health from a health-care professional. 1 of 6 children's records reviewed was missing the authorization to obtain emergency medical care and to transport the child for emergency medical treatment.

A wall was added in the ifant room. This change reduced the space in the infant room.

A careigver caring for a group of children did not have training in Pediatric first aid with rescue breathing.

A caregiver was caring for a group of children of varying ages including an infant. There was an 18 month age difference between the youngest and oldest child in the group. The youngest child was 13 months old and the oldest child was 3 years of age.

A group of children in the afterschool room were observed playing on tablets. The games the children were playing were not related to the planned activities.

Two toilets in a children's restroom were missing the toilet seat.

The director did not have current training in Transportation safety.

The emergency medical transport and treatment authorization forms were missing from the transportation binder for 4 children being transported. Note: This was CORRECTED during the inspection as the missing forms were obtained and filed in the transportation binder.

A children's toilet was missing the toilet seat.

3 of 6 children's immunization records reviewed were not current.

The thermometer in the first aid kit from the transportation vehicle was not working.

Trash such as a banana peel and plastic bag were observed in the outdoor play ground accessible to children. Cleaning supplies and plastic bags were observed under a children's sink. There was no latch on the doors preventing the cabinet from opening. Note: This was corrected during the inspection as the trash were removed from the outdoor playground and the cleaning supplies found under the sink were stored in an area out of children's reach.

There was no carbon monoxide detector at the center.

There was no documentation available for review as required for a unitary surfacing meterial in the outdoor playground.

The sign in and out log was not maintained as only 9 children were signed in but a total of 12 children were counted present. Note: This was CORRECTED during the inspection as the sign in log was updated with the missing information.

1 of 4 children's records reviewed was missing the parent's authorization to obtain emergency medical care and to transport the child for emergency medical treatment. 1 of 4 children's files reviewed was missing the child's immunization record.

The following items were missing from the f first aid kit in one of the transportation vehicles: 1. Thermometer; and 2. antiseptic solution or wipes

A caregiver caring for a group of children was missing Eight hours of pre-service training.

1 of 4 personnel records reviewed was missing the documentation showing how the caregiver meets the minimum age and education qualifications, A statement signed and dated by the employee verifying the date the employee attended training during orientation. A statement signed and dated by the employee showing he/she has received a copy of the child-care center s: (A)Operational policies; and (B)Personnel policies.

The director was missing 3 clock hours of annual training in management techniques, leadership, or staff supervision.

A caregiver with less than 90 days of employment was caring for a group of children without training in Pediatric first aid.

Personnel records evaluated. 8 out of the required 30 annual training hours were completed. One personnel file was missing Abuse and Neglect training.

Naptime observed, children were sleeping on mats close to one another where not enough walkway or space between children. A caregiver observed to have the need to step over and on mats to reach to a child on opisite end of room.

Activity plan for toddler room was not avaiable for review.

Nap mats observed to not be labeled with children names or any alternative labeling method such as a number list for each child or stickers on tags.
