A current health inspection was not available for review. Last health inspection was dated 9-15-23.

4 staff records were reviewed and are missing the following trainngs: 1) All annual trainings 2) Allergies 3) All annual training 4) All annual training

There were two groups of toddlers in an elevated table that were not securely strapped. The seats were missing the safety straps.

A teacher/caregiver hired on 9/2022 did not have the required 24 hours of Preservice training.

Three ceiling tiles in three classrooms were observed with water stains. Corrected at Inspection: The ceiling tiles were replaced during the inspection.

A teacher/caregiver completed pediatric CPR training online without hands-on skill-based instruction, and practice.

The sign in and out log was not maintained. Corrected at Inspection: The times and initials were placed for those who had not been sign in.

Three teachers were allowed to be present at the operation without a background check eligibility result. Note: The teachers were removed from the classrooms.

Based on the information gathered during the investigation, it was determined a child was unattended outside the classroom during drop off time.

Three teachers did not have the following training topics: - Preventing and responding to emergencies due to food or an allergic reaction; --Understanding building and physical premises safety, including identification and protection from hazards that can cause bodily injury such as electrical hazards, bodies of water, and vehicular traffic; and -- Handling, storing, and disposing of hazardous materials including compliance.

The operational policy was incomplete.

A child's Food Allergy Plan was not posted in the classroom. Corrected at Inspection: The Food Allergy Plan was posted during the inspection.

One personnel file did not have the following items: Affidavit, education, Orientation, and Identification.

One child did not have the physician health statement on file.

There were two unitary mats on the exit of the slides which pose a tripping hazard for the children during outdoor play.

Three staff did not have the required annual training topics.
