In the bathroom of the 2 year old room there was a crib in a bathtub that was on top of a box which was a safety hazard. In the bathroom of the 20-26 month old classroom there was ribbon, garland and cellophane paper in the bathtub that was a safety hazard. In the after school room the caregiver's purse was accessible to children. The Director corrected all of the hazards at the time of the inspection.

In two different rooms the diaper changing mats had tears in them. The Director corrected this at the time of the inspection by replacing one mat and covering the tears on the other mat with duct tape until a new mat is ordered.

In the 20-26 month old room the caregiver was not using the four step process for cleaning and sanitation. The Director corrected this by speaking with all of the staff and having the materials necessary to do the four step process.

There was on infant feeding instruction that expired on 7/1/2024. The person in charge is going to correct this by getting the feeding instruction updated.

A fire drill has not been conducted since 5/14/2024. The person in charge is going to correct this by getting a fire drill done.

The fire inspection expired on 3/28/2023. The Director is going to correct this by getting the fire inspection done.

Two areas of the playground did not have the required amount of mulch. The owners are going to correct this and up supervision until the loose fill is installed.

The diaper changing mat in the 22-24 mo old room had some tears. The Director corrected this at the time of the inspection by putting duct tape on the tears until she replaces the mat.

A fire drill has not been done since 4/7/2022. The Director is going to correct this by having a fire drill.

There were three infant feeding instructions that had not been updated. The Director is going to correct this by getting the feeding instructions updated.

There was enough evidence obtained to determine that a caregiver was not properly supervising children when she left a child alone in a room for approximately 7 minutes. The caregiver did not do a head count when leaving the room which caused the child to be left alone.

The Director let Licensing know that she knew about a hole in the fence for three days and did not mark it on the daily inspection form or get it immediately repaired. This resulted in a dog coming through the fence and jumping and biting at a child. The child was not injured.

A child was left on a sidewalk outside for a minute or less alone. The child was found unharmed.

A child was left outside for a minute or less and the parent was not given an incident report to sign within the Licensing time frame.
