A total of 21 children ages 11mo-4yo were combined in the dinning room, Age difference of the youngest and oldest child in the room exceeded 18 months.

There were 21 children in the dinning room ages 11mo-4yo, The TV in the room was on and playing videos and one child was watching videos in a tablet

Children in the dinning room waited 30 minutes with no activities while waiting for the rest of the breakfast to be prepared and served.

An employee was present at the operation without prior background check legibility notice. Note: This was corrected at inspection when provider sent employee home pending background check eligibility.

The operation moved the infant room to a room that used to be a toddler room prior to notifying Child Care Regulation.

A completed Children's Products Certification form was not available for review during the inspection.

A diaper changing pad in the pre-k room was torn. Note: This was CORRECTED during the inspection as the diaper changing pad was replaced with a new pad.

A wooden fence picket was broken in the outdoor playground. The broken area of the fence was within children's reach.

A caregiver was left caring for 6 infants in the infant room when the second caregiver was observed outside of the center waiting for a school aged child being dropped off by the school bus. The ages of the children in the group were a 4 month old, a 6 month olds, a 10 month old, two 11 month olds, and a 12 month old. According to the ratio chart the maximum number of children one caregiver may supervise is 4. The group was out of ratio by two childlren.

Ratios were not met in the cafeteria/multi-purpose room and the infant room. A caregiver was caring for 16 children of varying ages in the cafeteria/multi-purpose room. The specified age of the children in the group was 2 years of age. According to the ratio chart the maximum number of children one caregiver may supervise is 11. The group was out of ratio by 5 children. A caregiver was observed caring for 5 infants in the infant room. The ages of the children in the group were a 4 month old, a 6 month olds, a 9 month old, and two 10 month olds. According to the ratio chart the maximum number of children one caregiver may supervise is 4. The group was out of ratio by one child. Not: A second caregiver stepped into the room during the inspection.

A record was not available for review for a child in care.

The center did not notify Child Care Regulation about providing transportation services to chidren in care.

Infants were served the afternoon snack on a bare table. Note: This was addressed with the caregivers and a napkin was provided to place the snacks on.

The operation staff interfered with the inspection when staff did not open the door for the inspector to enter the operation until after 8 minutes of knocking, ringing doorbell and calling the operation's phone number.

The person in charge of the center in the director's absence did not designate a second person in charge when they left the operation for their lunch break. When caregivers were asked who was in charge they provided a name of a staff member that was not present in the operation.

3 of 5 children's files reviewed were missing the authorization to obtain emergency medical care and to transport the child for emergency medical treatment, and a statement of the child s special care needs.

The arrangement of the mats during nap time were positioned very close to each other and did not provide a walk and work space for caregivers between mats.

A group of children including children under the age of 2 were watching television while waiting to be served their lunch.

The safety straps were not fastened for an infant using a floor bouncer.

The operational policies reviewed were missing the following information: (A) The benefits of physical activity and outdoor play; (B) The duration of physical activity at your operation, both indoor and outdoor; (C) The type of physical activity (structured and unstructured) that children may engage in at your operation; (D) Each setting in which your physical activity program will take place; (E) The recommended clothing and footwear that will allow a child to participate freely and safely in physical activities; and (F) A plan to ensure physical activity occurs on days when extreme weather conditions prohibit or limit outdoor play.

Two cribs were observed without a tight fitting sheet covering the mattress pad. The cribs were currently being used by children.

An infant was observed asleep on a restrictive device for an undetermined amount of time. Note: This was addressed during the inspection and the infant was moved to a crib.

A 7 month old child was observed to have a nursing pillow placed under them while sleeping in the crib.

The ceiling in the pre-k room was observed with water damage. Several ceiling panels were observed with Yellowish-brown stains and black mold. The wooden fence in the outdoor play ground was not maintained. Some parts of the wooden fencing were observed with peeling paint accessible to children. NOTE: The ceiling panels were replaced during the inspection.

Child/caregiver ratios were not met in the infant room when a caregiver was caring for 5 infants. The specified age of the children in the group was 8 months of age. The child/caregiver ratio for children between 0-11 months of age is 4 children to one caregiver. The classroom was out of ratio by one child.

Based on the information gathered during this investigation, a caregiver left a group of children unsupervised when the caregiver in charge of the group stepped out of the room to go open the center's front door.

Based on the information gathered during this investigation, staff members demonstrated poor judgment when they served food that was too hot for young children. As a result of this, a serious injury was sustained by a one-year-old child.

The operation allowed a dance instructor to be regularly present at the operation and have direct access to children in care, prior to receiving notification of the dance instructor's background check eligibility.

A large branch that broke off a tree was observed in the children's outdoor playground accessible to children.

A caregiver was caring for a total of 7 infants in the infant room. The specified age of the children in the group was between 12-17 months. According to the ratio chart the maximum number of children a caregiver may supervise between the ages 12-17 months is 5. The group was out of ratio by two children. Note: A second caregiver stepped in the classroom during the inspection.

Three floor mats were not labeled with each child's name in the infant room. NOTE: This was CORRECTED during the inspection as the floor mats were labeled with a number and a number/child assignment list was created.

The emergency preparedness plan reviewed was missing the specific procedures for evacuating and relocating children who are under 24 months of age, who have limited mobility, or who otherwise may need assistance in an emergency, such as children who have mental, visual, or hearing impairments; and How children in attendance at the time of the emergency will be accounted for at the designated safe area or alternate shelter.

Four cribs and a play yard did not have a fitted sheet covering the crib's mattress. Note: Sheets were provided for the four cribs.
