At the time of the inspection 1 of the 6 children's records reviewed did not have an accurate birth date recorded for the child.

At the time of the inspection 6 of the 6 children's records reviewed did not have the name, address, and contact number for the child's physician or an emergency medical facility.

At the time of the inspection a child with a noted food allergy on their admission form did not have an allergy action signed by their physician on file.

At the time of the inspection the operation had not conducted fire drills for 2 of the last 5 months.

At the time of review 1 employee had incomplete training records. The remaining required hours were stored in a manner that made them inaccessible at the time of the visit.

At the time of the inspection it was found that a child has an epi pen stored at the operation in a ziploc bag with the child's name on written on the bag. The epi pen itself has no identifying information for who it was prescribed to and the prescription box for the epi pen was not on site at the operation.

Based on information obtained the operation did not obtain ID and record the identity of an individual picking a child up from the operation who had not picked up the child before.

Based on information obtained caregivers released a child in care to their sibling, who was under the age of 18. In doing this the caregivers did not check enrollment documents to verify information of individuals giving permission to release the child and they did not check ID or record identity verification of the sibling picking the child up. All of this violates the operational policy for release of children.

This standard was evaluated in regards to a report and was found to be deficient. The operation was aware that an employee tested positive for COVID-19 on January 21, 2022 . The report was not made to Licensing until February 1, 2022.

The program allowed a person to be present at the operation before receiving notice from the CBCU regarding the person s eligibility. The operation submitted a background request during the inspection.
