All booster seats in both transports were expired since 2015-2017.

Director did not have a current copy of the performed fire marshal inspection.

A caregiver responsible for a group of children was unaware of the number of children responsible for. The caregiver stated 12 children were present however 11 were counted. A caregiver responsible for a group of children age 5-9 years old was unaware of the number of children present. The caregiver stated 14 children were present however 13 were counted.

A food allergy emergency plan signed by both parent and physician is needed for one child enrolled.

Updated immunization records are needed for three children enrolled.
