The annual sanitation inspection was expired.

The changing mat in the two year old classroom was ripped. Note: changing mat was replaced during this inspectiion.

One child epi pen was expired.

The incident/illness report form was not completed within 48 hours of the incident.

One infant did have current infant feeding.

Based on the findings from a DFPS investigation caregivers did not demonstrate good judgement when not asking for assistance when needed which resulted in a child being abused as well as a caregiver not intervening when the abuse occurred.

Based on the findings from a DFPS investigation, corporal punishment was used on a child in care when the caregiver picked up a child by one arm in an aggressive manner and resulted in physical abuse.

Based on the findings from a DFPS investigation, an incident report was not written informing the parents that the incident occurred.

Based on the findings from a DFPS investigation, one caregiver was found Reason To Believe for Physical Abuse and another caregiver was found Reason To Believe for Neglectful Supervision.

One of the caregivers had completed CPR training through online instruction and did not attend a class.

The diaper changing mat in the Oaks classroom was torn and as a result could not be properly sanitized. Note: A picture was taken. Note: The mat was fixed during the inspection.
