During the course of the investigation, it was reported that caregivers have grabbed children by the wrist when moving them to another location for time out.

During the course of the investigation, it was reported that caregivers at the operation yell at children in care.

The fire extinguisher in the van had not been serviced in the past year. This was corrected when a new fire extinguisher was purchased and placed in the van.

Four children's files did not contain an address for the emergency contact listed. This was corrected when the addresses were added to the contacts.

One employee file did not contain SIDS and Shaken Baby Training, that cares for children under 24 months of age.

A review of the Employees Who Left Employment in 2021 data from February 23, 2022 revealed that the center had not reported the number of employees who left employment in 2021 by February 22, 2022 as required.
