There were 2 infant feeding instructions that were observed to not be up to date.

A caregiver did not have a notarized affidavit on file for review.

A caregiver did not have documentation of orientation completed in their file.

7 out of 7 personnel files were reviewed. One caregiver had only completed 10 out of 24 hours, another caregiver completed 7 out of 24 hours, another caregiver completed 4 out of 24 hours and another caregiver had only completed 6 out of 24 hours.

In the toddler classroom, under the changing pad, there was dirt and debris observed. In the bathroom, there was toilet paper observed all over the floor and the toilet had urine in it. In the playground there was a hole that presented a tripping hazard.

In a classroom, there was trash observed on the floor, the trashcan was observed to not have been emptied. The furniture in the classroom was observed to be broken, there was an exposed screw and one of the shelves was leaning. In the playground, there were 2 large holes and a torn mat where the step down is that presented tripping hazards.

The operation did not have a current fire inspection completed.

A caregiver stated she had 17 children in care and it was determined she had 18 children in care.

The classroom was observed to have 18 children with 1 caregiver, the age specified age group was 3 years old, the classroom was over ratio by 3.

The classrooms were observed to not have been swept, there was debris under the changing mats in both of the infant classrooms. There was food observed left out. There was a fountain drink observed in a classroom that was not being used. The blankets were observed to have been tossed together. There was feces observed on a bathroom stall and dried urine in the corner of another stall. The cabinet in the bathroom was observed to be dirty. Each door in the facility was observed to be dirty. the safety straps on a infant's changing table were broken off. The grass has not been cut and there were several grass clusters observed on the playground.

The last sanitation inspection was completed on 8//29/24. The operation did not have an updated inspection on file.

Based on observations, the bucket seats for the younger children were observed to not have been wiped down and crumbs were visible in the bucket seats.

Based on the observations from current inspection, it was determined the directors were not ensuring the operation is operating withing compliance of the minimum standards.

There were 3 outlets observed without an outlet cover.

It was determined a designated director has not been at the operation consistently for 75% of the time.

A bottle of apple juice that was observed to be half way utilized was observed sitting in a classroom and was not placed back in the fridge the day prior. The classroom the juice was in was not being utilized during the inspection.

The walls and floors in the operation were observed to be dirty.The walls had paint chipping off in some sections and had markings throughout the wall. The floors were observed to not have been swept and were sticky in a classroom.

A caregiver was unaware of the ages of the children in her care. The documentation posted did not have the correct ages of the children in the classroom.

There was no documentation that the fire extinguishers were being checked monthly.

The smoke and carbon monoxide detectors were not documented to show they are checked monthly.

It was determined fire drills were not being completed monthly as documented.

There were 6 infants observed with 1 caregiver. The age specified group was 0-11 months, bringing the classroom out of ratio by 2.

A fire inspection had not been completed. The last fire inspection was completed on 8/23/24.

During naptime, there was not an additional caregiver in the building, that was not counted in ratio.

The operation had a swimming pool that was included in a big water slide inflatable and the children who could not swim, did not have a life jacket for use.

The large water slide that was utilized at the operation was not manufactured for commerial usage.

It was determined the operation did not have any life saving devices while children utilized a large water slide.

Based on the preponderance of evidence, it was determined the Director was not ensuring the operation was operating in compliance within minimum standards.

Based on the preponderance of evidence it was determined the Director was not present at the operation a minimum of 75% of the time.

It was determined during the Reptile Day older infants were grouped with School Age Children, that had more than an 18 month age gap.

It was determined the operation had a Reptile Day at the center and the children were exposed and were able to pet iguanas and snakes.

It was determined the operation was having children sit in time-out for a few minutes, as the remainder of the classroom went out to the playground.

During the inspection, soiled diapers were observed in a trashcan with no lid. The caregiver removed the trash during the inspection.

Screen time was observed being utilized in 2 classrooms during snack time. The caregivers turned off their televisions during the inspection.

Two children were observed not fastened in a chair that was manufactured with fasteners. The caregiver removed the children from the chairs during the inspection.

A caregiver stated she had 9 children in care and it was determined she had 8 children in care.

The operation did not utilize good judgment when the sent a notification to parents of the operation, less than 20 minutes before the bus for the ISD was set to arrive, of the operation closing early.

Based on the information received, it was determined a caregiver had completed online CPR instead of in person with use of a manikin.

It was determined the operation was exceeding the max group size for the classroom by having over 35 children present at one time.

During the inspection, 2 children were observed with a blanket over there head during naptime.

A caregiver counted in ratio was observed answering phone calls for the operation.

A caregiver informed she had 18 children in care and it was determined she had 20 children in care.

3 out of 5 caregiver personnel files did not have Affidavit Form 2912 on file for review.

An infant was observed sleeping on the carpet/floor during the inspection. The infant was moved to their crib.

The operation did not provide documentation that a caregiver had completed her CPR training in person.

The operation did not provide the educational requirements for 1 caregiver.

There were 11 infants with 2 caregivers. I infant was placed in the second infant classroom.

The cots and mats were observed to not have been labeled.

The total capacity for the operation is 73 and the operation had 78 children in care. The operation's capacity for 18 months and older is 51 and the operation had 66 children in care that were 18 months and older. The children were sent home during the inspection by the operation.

On the playground there was a 12 month old infant with a 3 year old present on the playground. The younger classroom were brought inside.

A personnel file was not available for review during the inspection. A caregiver's CPR/First Aid Certification was not available for review during the inspection.

There were 5 children that did not have a mat or cot to sleep on. The Director provided the children extra mats during the inspection.

A caregiver was observed sweeping during naptime, while several children were walking around and the other caregiver was attending to a child. During the inspection, the director informed the caregiver to assist with naptime.

The operation did not provide documentation of orientation completed by the caregiver.
