During the inspection, the outlets in the 2 year old classroom was not covered.

During the inspection, there was 1 caregiver with an expired pediatric first (10/2025). Caregiver also had expired pediatric cpr (10/2025)

During the inspection, the operation did not have the children emergency medical informaiton for children that are being transported.

During the inspection, there were 2 bottles that were no labeled.

During the inspection, the emergency prsctive drilld were not documented correctly. The operstion did not complete a fire drill for January 2026. For the year 2025, the fire drills were missing the start time. The sheltering in place, severe weather and lock down drills were missing the start time and exit time.

During the inspection, children files were viewed and four out of ten files viewed were missing the emergency medical information.

During the investigation inspection, there were 3 bottles not labeled with the child first name and last initial. Note: This was brought into complaince when caregiver labeled the bottles with the child name.

During the follow up inspection, director is ensuring the building ground checklist is being used when being checked. Playground is being checked in the mornings daily and at the end of the day.

Director advised conversations has been had with the caregivers and is ensuring good judgement is being followed for all the children.

During the follow up inspection, it was stated the caregiver is still working at the operation once she leaves school and is working with another caregiver.

2 out of 11 files did not have the required notarized affidavit available for review.

A review of the Employees Who Left Employment in 2024 Survey data from January 17, 2025, revealed that the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.

The annual report has not been done for this year. The last report was 05/22/2023.

The director did not have the required transportation training.

The van used to transport children in care did not use the booster seats as required. Photos were taken of each row of the van without any booster seats. The director was able to put the booster seats back in the van.

In the toddler classroom, the diaper changing mat was torn.

An infant was placed in a highchair without using the safety straps as required.

A caregiver hired on 10/02/2023, who was present in the classroom with children, did not have a background check results on file.

Based on information obtained during the investigation, the parents were not notified of a child becoming ill while in care. The child's mouth area was blue, and the parents were not notified of it.

The Operational policy or the Parent handbook was missing several sections.
