A caregiver failed to do a proper attendance check of children in her care while transitioning from one classroom to another. She also failed to identify the issue on her own.

A child was left completely unattended and unaccounted for within a classroom for up to 30 minutes before being discovered.

Five children have expired feeding planes on file at the opeation.

The operation's Infant class had five children in care with one caregiver. Based on the ages of the children in care the caregiver can only care for four children.

Five children files are do not have immunization records. All children files reviewed a missing important required information.

Two caregivers do not have pre-service training on file at the operation and have been present for over 90 days.

One caregiver has been working at the operation since 2022 and does not have a CPR and First Aid card.

The operation does not have documentation of gas leak inspection. The latest documentation file is from three years ago.

There are 2 playgrounds that do not meet minimum standards for surface levels. 30 days will be given in order to have both playgrounds at adequate surface levels for children.

There are 3 areas in the combined playground area that have exposed nails and sharp edges on the border that holds the wood chips for ensuring soft play surfaces.
