Teacher left the classroom to cnswer the front door, then went to another classroom to notify the director that someone was there to see her.

Child's medication was not in the original container labeled with the child s full name and the date brought to the operation.

Two children ages 16 and 17 months were observed sitting in high chairs without an activity. One caregiver was observed patting a child to sleep while the other was observed playing with two infants that were awake in a center. When brought to the attention of the director, the children were taken out of the high chairs and given an activity.

There was an infant that was swaddled in the crib, without having a Sleep Exemption Form on file.

There were 5 children that were sleeping that had their heads covered by their blankets. Note: This was corrected at inspection when the caregivers in the room quickly uncovered all the children's heads.

There were three children files out of the six viewed that did not have a signed health statement from their health care physcian in their file.

The loose fill around the slide exits were not the required 6 inches in depth. The ground was visible in these areas around the slides.
