This standard was tasked over for this investigation and found to be deficient. It was determined that a custodian of the child in care who is granted rights to the child's educational record by court order was denied access to the child's file. This information was requested multiple times.

Based on information provided from a DFPS investigation it was determined that a child in care was abused by a caregiver and the caregiver mentioned in the allegations has been found Reason to Believe.

Based on information provided from a DFPS investigation it was determined that a caregiver grabbed a child by the arm and lifted them from the ground. The action by the caregiver caused substantial bruising to the child's arm.

Based on information provided from a DFPS investigation it was determined that a caregiver did not utilize positive statements when redirecting a child in care to move to another area in the room.

This standard was tasked over for this investigation and found to be deficient. It was determined during this investigation that the child mentioned in the report was not given and/or offered water or a change of milk to drink through the school day. The child was allowed to only drink the same milk he arrived to care with throughout the day. the center director, has in place that the children's personal cups can only be filled with water at drop off time. Milk will be given throughout the day in disposal cups.
