A caregiver was present in the classroom caring for children without an active background check on file. The staff was sent home during the inspection.

Notification was not provided to the parent by pick-up time concerning an incident that caused the child's teeth to become chipped.

Based on the DFPS investigation, a caregiver did not use appropriate redirection as the caregiver grabbed a child by the wrist and lifted the child from a seated position to direct the child to the sink.

The operation did not have a background check submitted for a caregiver working at the center. The background check was ran while at the inspection and the caregiver was sent home.

One caregiver was missing an up to date CPR/First Aid training.

During the inspection, there were infant bottles observed with the first name and no last initial.

It was determined that a caregiver did not use good judgement when not checking the bottles to verify the name, which resulted in an infant being fed the wrong bottle.

The employee list was observed posted but was not updated with the current staff. The list was last updated on 1/25/25. This was corrected at inspection when an updated employee list was made and posted.

It was determined that the operation did not report to CCR that a caregiver gave an infant another infant's bottle.

It was determined that an incident report was not signed by the parents after a child was injured while in care.

It was determined that the staff did not correct a pattern of behavior when a child continued to climb up the slide and jump off, which caused the child to injure themselves.

Three staff are missing the annual 24 hours for the last full year.

Three caregivers witnessed an incident where a child required medical attention, and did not adequately intervene.

Based on evidence obtained during the investigation, it was determined that a caregiver grabbed a child by the arm as a form of redirection rather than using positive statements.

A 3 year old child was left unattended in the classroom for approximately 8 minutes.

A caregiver did not demonstrate good judgment when not correcting children on how to properly use the outside play structure equipment.

During the inspection, three staff did not know the childrens ages that they were caring for and were unable to obtain the information.

An infant was observed sleeping in a swing.

The 11 months to 24 months class was observed outside on the playground out of ratio. There were 2 caregivers and 14 children with a specified age group of 11months to 18months.

It was determined a child was able to get out from the playground gate and walked to the front of the building. The child was unsupervised for approximately less than five minutes. The child was found by a parent.

A child was placed at risk when being able to get out from the playground gate and walk to the front of the building and was left unsupervised. The parents were not notified until 2 hours later.

Based on information obtained it was determined that three classrooms were out ratios for more than 30 minutes. The three classrooms out of ratio were Room 6, Room 7, and Room 8.

Based on the information obtained, classrooms were out of ratio while the owner was in charge of the center.

The operation allowed a caregiver to have access to children without obtaining the results of the background check. The caregiver was sent home.

The operation did not have an incident report for a child being left unsupervised in a classroom. The operation did not have incident reports for a child having two seizures at the center.

During interviews, it was determined that a child was left unsupervised in a classroom.

One caregiver does not have current training in first aid since October 2021.

Based on the evidence obtained, it was determined that the director has lied about incidents occurring at the operation such as children having seizures, children being injured in care, and children being left unsupervised.

The operation did not report to Child Care Regulation that a child was left in a classroom unsupervised.

On two separate occasions a child had a seizure and the operation did not contact EMS for the incident.

One caregiver does not have current training in CPR since October 2021. In addition it was determined that three staff were supervising children alone while not having CPR training.

A caregiver in the infant classroom was observed not washing hands after diapering a child.

The 2 year old class was observed out of ratio outside with 1 caregiver and 12 children.

The operation did not use the proper cleaning solution when cleaning the floors. The product used was not designed to be used on flooring.

During an investigation inspection on 2/22/2022 an infant was observed sleeping in a crib with a pacifier clip connected to the shirt.

During the course of an investigation, video footage was observed and a caregiver did not wash their hands after changing children's diapers.

During the course of an investigation, video footage was observed showing children in a classroom not washing their hands after having diapers changed.

The facility did not report to licensing that a child received medical attention for a rash.

An infant was observed asleep in a crib with an inapproriate sleep sack that restricts the arms and the legs from moving. The sleep sack was removed off the infant.

Two staff did not have a submitted background check and were present at the operation. The staff left the operation.

One infant does not have written feeding instructions on file and seven infants do not have updated feeding instructions.

A caregiver without a complete background check had access to children. This was addressed with the director and the caregiver left during inspection.
