The daycare is missing current fire marshal inspection last one expired November 2024.

I observe caregiver supervising a group of nine children in the toddler room missing a background check. NOTE: The caregiver left premises during inspection.

Three staff files evaluated were missing one hour of child maltreatment training.

During follow-up inspection on 4/24/2024, it was determined the background check for two caregivers present and working with children were inactive.

I observe various training cups that were not labeled with child's name.

Based on the information obtained during investigation, it was determined that caregiver did not use good judgment when a child was fed the incorrect bottle.

I did not observe current fire marshal's insepction it expired on 10/31/23.

First aid kit located in daycare bus were missing thermometer.

Two caregiver's supervising children were missing current first aid training.

Four caregivers records evaluated did not have current abuse neglect training.

I observe one of the gutter downspout loose with sharp edges accessible to children in the playground.

Based on the information obtained during the investigation, it was determined caregiver did not keep hands on the child at all times during diaper changing as required.

Based on the information obtained during the investigation, it was determined caregiver was not within child's physical proximity when she left the diaper changing table to retrieve gloves leaving child unattended for over one minute.

During the inspection I observed an infant placed in a highchair without the safety straps fastened as required.

During the walkthrough I observed bottles of cleaning supplies hanging from the cabinet doors in two classrooms and cleaning supplies in unlocked cabinets accessible to children in a third class. This was corrected when the caregivers removed the cleaning supplies and placed the products out of reach of children.

It was determined that the operation was reducing ratio during naptime for children under 18 months.

I observe six children ages 13-months to 15-months with one caregiver during naptime. The classroom was out of ratios by one child. Note: corrected at inspection.

The director is missing current CPR/FA training.

One caregiver's record evaluated was missing personnel and training information.

The activity plan was not posted in several classrooms.

The sleeping cots were not labeled with child's name or other identifying information. Note: corrected at inspection.

I did not observe current annual fire inspection.

Based on the information obtained, it was determined caregiver did not count the number of children before going outside and did not notice two children left in the classroom.

Based on the information obtained, it was determined caregivers left two children unsupervised in the classroom during naptime.

Based on the information obtained, there was sufficient evidence to support good judgment and self-control were not used when a caregiver was observed on video yelling at a child.

The infant feeding instructions for one infant was not available for review during the inspection.

The parents pick the child up and the injury was not noticed. The parents checked the child later and discovered the swelling. The childs leg was sprained.

The caregiver was not truthful with her assessment of the incident. The caregiver did not follow the safety rules put in place by the operation to maintain safety in the infant room. Evidence was verified on video.

The parents picked up the child at 12:30, they were never informed about a possible injury. The parents checked the child at 3:30 or 4pm and discovered the swelling. The childs leg was sprained.

A one year old did not received the proper first aid after an injury occurred which resulted in a sprain.
