CHILDRENS LIGHTHOUSE LEARNING CENTER AT SCHERTZ

During inspection, a caregiver who did not have an active background check was present in a classroom. This was corrected at inspection when the caregiver left the operation.

During inspection, cots were labeled with numbers however the assigment list indicating which child is assigned to each cot was not followed for four of the classrooms.

1 out of 10 caregivers' records reviewed did not have the required 24 training hours for the last full year of employment.

Director's annual training required additional topics were not available for the last full employment year.

At time of inspection medication log was not filled with date, time, dosage and staff who administered medication. This was corrected at inspection.

The 18-24 month classroom was found out of ratio.

Infant classrooms crib sheets were noted loose and not snugged.

A toddler classroom was noted with a nap mat blocking the emergency exit.

Two classrooms were noted with electrical cords accessible to children's reach.

Three classrooms were observed with nap mat placed right next to one another.

The operations health inspection was found outdated. The expiration date was 10/28/2021.

Re-cite: One out of the two missing immunizations was not ready for review.

Four out of ten staff records were not ready for review due to reorganization. That included the director's file.

9 out of 10 staff files did not have a first aid and CPR card training ready for review.

A staff member's purse in the two-year-old classroom was sitting on a shelf accessible to the children in care. Note: This was corrected at the inspection.

Two children's files reviewed did not have a current immunization record for review.

Emergency preparedness plan was found missing how staff will ensure care for children in the new evacuation location.

One out of ten children's files was found with incomple information on the authorization to obtain medical care.

A caregiver in the 3 year old classroom was overhead telling a child, "You better sit down," in a loud voice. This was corrected when the person in charge went into the room to assist the caregiver and discussed positive methods of redirection.

In the kitchen of the operation two sanitizing sprays were found in the food prep counter accessible to food. Note: Both sprays were removed during the inspection.

In the 2 1/2 -3 year old classroom, children were observed out of controlled.

During lunchtime, children in the three year old classroom were observed and did not wash their hands before eating.

Operation policies were missing the time durations on how the operation will be promoting indoor and outdoor physical activity.

The nap mats were not individually labeled with the children's names. The nap model arrangement was not being followed in two pre-k classrooms.

The outdoor play fence was observed with a large gap big enough for a child to escape out of. The temporary blocks are not adequate enough to ensure the gap is covered at all times.

An electrical outlet outside was missing a cover. This was exposed to all children when they go outside to play. This was corrected when a staff placed covers in each outlet.

The carbon monoxide detector that is located in the director's office was not being checked and documented.

The last fire inspection was conducted on 1-29-2020. Fire inspection are due annually.

All tricycles that were found on the playground were missing the caps on the end of the handlebars and cracked seats which were sharp to the touch for children. A few were also missing peddles. This was CORRECTED when the tricycles were removed off the playground.

The three vans did not have written plan to handle transportation emergencies located in the binders. This was corrected when the director printed it off from the transportation training to put in the binders.

There was no documentation for a fire drill being conducted in the month of November. The office staff conducted one on November 29th while the director was off but it was not documented on the form.

One out of three vans did not have a complete first-aid kit. It was missing a thermometer, guide, and gloves. This was corrected when these missing items were placed in the van.

The fire extinguisher located in the kitchen had a tag showing that it was last serviced in February 2020.

The periscope found on equipment on the playground was observed to be loose resulting in an opening in which a child's finger was trapped. Note: The equipment was removed from the playground immediately following the injury and does not currently pose a risk to children.
