Two medictions stored at the operation did not contain the original prescription for the medication.

An over the counter medication kept at the operation for a child enrolled expired 11/2024.

One out of ten staff training files evaluated did not have documentation that a caregiver without 2 yrs prior work experience completed the first 8 hours of pre-service training.

Two classrooms did not have current activity plans posted. The last posted plan was dated 8/5.

The most recent fire inspection report was not available for inspection. The last recorded inspection was held 8/15/23.

The operation failed to provide a incident report or information regarding an injury a child received while in care.

The slide on the climbing unit continues to be roped off and has not been repaired or replaced. Exposed tree roots have not been removed or covered from the playground. NOTE: This is a recite. Documentation was provided that the operation is in the process of purchasing a replacement slide with the playground vendor.

A television in the dramatic play area was at the children's level and not anchored to avoid possibly being pulled down off the shelf.

The operation did not have a current sanitation inspection available for review. The last recorded inspection was conducted on 1/10/23.

The slide on the climbing unit is currently taped off at the top and bottom of the slide. The bottom of the slide has two broken areas where children can be injured or clothing could potentially get caught on. There are also numerous tree roots exposed on the larger playground that can cause a tripping hazard.

A caregivers purse was out on the floor next to a sleeping child. NOTE: The caregiver placed their purse in a cabinet during the inspection.

A caregiver was observed using their personal tablet while supervising children during nap time. NOTE: Caregiver put tablet away during the inspection.

2 out of 10 staff did not have a current 1st Aid certificate on file.

7 out of 10 caregivers had not completed training in the following topics for the anniversary years evaluated: emergency preparedness; preventing and controlling the spread of communicable diseases, including immunizations (this requirement was previously an optional topic); administering medication; preventing and responding to emergencies due to food or an allergic reaction; building and physical premises safety; and handling, storing, and disposing of hazardous materials.

2 out of 10 caregivers did not have a current CPR certificate on file.

The operation ordered takeout food to serve the children for lunch when the cook was absent and did not include additional food components in order to follow the USDA meal patterns.

The operation did not keep record of food substitutions to their weekly planned menus.

1 out of 10 child files evaluated was missing updated vaccination records for a child who was 16 mos of age. The child was missing their MMR & Varicella vaccinations.

2 out of 10 child files evaluated did not have a health statement signed by a doctor after being in attendance for 12 months.

A church member had direct access to children ages 13-16 months of age without an eligibility letter on file. NOTE: A background check was submitted through the operation's portal during the inspection.

Cleaning bottles were not properly labeled with the contents contained and as a result, bleach water was sprayed on a child's hair.

The operation failed to notify Licensing that a child was potentially placed at risk of injury when bleach water was accidentally sprayed on the child's hair.

The incident report was not signed by a management person at the operation until 7 days after the incident report was completed.

3 out 10 child files reviewed for immunization records were missing updated immunizations.

An infant, 8 months of age, was observed sleeping in their crib with a blanket.

A caregiver in the infant room was present and in direct care of the children without an eligibility letter on file. NOTE: The caregiver was removed from the operation and a background check will be submitted by the director.

Two Prek classrooms were observed not having adequate lighting to allow caregivers to see the children while sleeping from the opposite side of the room. NOTE: Caregivers opened a window blind to allow natural light to come into the room.

A caregiver in the infant room was present and in direct care of the children without an eligibility letter on file. NOTE: The caregiver was removed from the operation and a background check will be submitted by the director.

Two infants were observed sleeping in a swing. When caregivers were asked how long the infants had been sleeping in the swings, it was said to be 10 minutes. NOTE: Caregivers moved the children to their assigned cribs.

The operation did not notify a parent that their child received a minor injury received while in care during naptime.

Two out of ten child files evaluated were missing a health statement from a physician after being in attendance for 12 months.
