During a review of the operation's records of monthly inspections of active play space and equipment, I found the last recorded montlhy inspection was recorded on 07/08/2025.

During inspection, a crib assigned to a four month old infant was found to have a wedge under the mattress.

During inspection, an electrical outlet n classroom #2 (PreK 4) was found without a safety cover installed. The caregiver found a safety cover and installed it into the outlet during inspection upon request.

A four month old infant has a wedge under the mattress in their crib without having a Sleep Exception From signed by a health-care professional.

During inspection walk through, cleaning and sanitizing supplies were found accessible to children in classroom #4, a restroom used by children across the hall from classroom #4, and classroom #6. Staff members moved all the sanitizing products to areas that were inaccessible to children upon request by CCR inspector.

All caregivers have expired CPR. Note: Their CPR expired at the end of October.

I observed two caregivers on the playground not supervising children which resulted in two children eating dirt.

Based on information obtained during the investigation, there is sufficient evidence to support a citation that a caregiver was not able to intervene when a child put medication in their mouth that belongs to the caregiver.

I observed a 14 month old infant sleeping with a sippy cup. A photo was taken.

For the months of February and June 2023 the fire drills exit times were not less than 3 minutes. A photo was taken of the fire drills.

I observed a water hose on the ground in which children had access to it. A photo was taken and the director removed the waterhose.
