A teacher in room 7 was caring for three year olds without an eligibility letter.

Four out four employees records are missing preventing and responding to emergencies due to food/allergic reaction and building and physical premise safety.

In classroom 6 a plastic chair, couch and bookcase were blocking 2 children that were napping. Supervision was being hindered.

A teacher was left alone in the classroom. She does not have her eligibility letter. The provider reran the background check.

The fire inspection expired on 10/17/23.

The current health inspection expired on 08/31/23.

Three van drivers have only completed one hour of training.

Based the information gathered staff did not report suspected abuse. Several staff stated during their interviews they had concerns with the teacher yelling at children but did not report it.

Based on the information obtained during a DFPS investigation, there is sufficient evidence to validate that a child was forced to sit away from the group for more than one minute per their age.

Based on the information gathered children were being yelled at by a teacher.

The fire inspection expired on 08/13/22.

Lysol cleaner was found on the bottom shelf in a cabinet within reach of children. This was corrected during the inspection when the cleaner was removed and put in an area children cannot access.

During nap time a child's cot was placed behind a bookcase where the child could not be adequately supervised during nap time. Follow-up inspection on 03/02/222 showed a child was placed behind a book shelf and another child could not be seen where the teacher was sitting.

There is no documentation showing the smoke detectors have been tested for the months of January and February.

There was trash on the floor on bus one. This was corrected during the inspection when the trash was picked up by the Director.

There is no documentation showing the carbon monoxide detectors were tested for the month of January and February.

During nap time a child's cot was placed behind a bookcase where the child could not be adequately supervised during nap time.

There is no documentation that the fire extinguishers have been checked for the months of January and February.

One out of ten children's records is missing the child's doctor's information or nearest emergency care facility.

During the walk through 2 teachers did not know the number of children in their care.

The first aid kit was missing scissors. This was corrected during the inspection when the scissors were placed in the first aid kit.

A teacher left alone with children does not have her eligibility notification.
