Evident found that the director did not make a good judgment when taking her sick child to the operation, left him alone in a classroom and breakroom for more than one hour.

A child was on his tablet for more than one hour at time while in room 10 and staff's breakroom.

A six year old child was by himself in a classroom and staff's breakroom for more than an hour.

Based on observations made at the operation, supporting documents and interviews with various people, this standard is found to be in noncompliance.

Based on information received during investigation, it was determined that a caregiver was using candy bar as reward when playing musical chair.

A child got out of the classroom without caregiver noticed.

There was hand sanitizer within reach of children that was not in use in the school age children.

There was medication for 1 child that is no longer in care. The medication was also expired.

After changing a diaper and all clothing, a child had smeared feces on his hand.

A caregiver was observed to be frustrated during naptime, as several children were awake and she was unable to complete required clerical tasks. The director was not aware of this, and consequently a child in care was handled inappropriately.

A caregiver did not demonstrate self-control as she was observed grabbing a child by the arm, and forcefully placing the child on their nap cot.

Five children were observed laying awake near the end of nap time without an alternate activity provided.

Two classrooms did not have enough popular toys and activities that provide opportunities for thinking skills, sensory development, small and large muscle development, language and social and emotional development.

Upon observations of the toddler classrooms, lack of age appropriate equipment was noted. The children were also observed playing with magnets, which are suitable for children three years of age and up.
