During the walk through an uncovered electrical outlet in the 18 to 24 month classroom was observed in plain sight. The director corrected the issue at the inspection by placing a safety cover on the electrical outlet.

While reviewing personnel records, it was determined that a caregiver had completed a CPR course online that did not include a hands on component to the training.

While reviewing personnel records, it was determined that 4 of the employee folders were missing photo identification. The director corrected this at the inspection by obtaining copies of all four employees photo identification and placing them in the employee folders.

This standard was found to be deficient as the operation lacked infant feeding forms for five children in care.

The operation did not submit the information as required by January 15th.

This standard was found to be deficient after the operation was not able to provide a current Fire inspection.

This standard was found to be deficient as the operation did not report a positive Covid19 case to State Licensing in the required time frame.

This standard was found to be deficient after it was confirmed that there were issues with children biting that required increased supervision. Caregivers are not able to intervene to ensure the safety of the children in care as multiple children have been bitten in the classroom.

This standard was found to be deficient as a caregiver admitted to yelling at children when she would see that a child was trying to bite another child in care.

Four infants did not have updated feeding instructions.

A caregiver did not have First Aid training on file.

The standard was evaluated with regards to an investigation and was not compliant. A caregiver did not demonstrate good judgement when handling a child. The caregiver dragged the child that was laying on floor by the forearm from a learning station that was in close proximity to a diaper changing station. Dragging a child that is laying on the floor by the forearm could result in a child sustaining an arm injury.

Based on the interview with the victim, the operation was not found to be in compliance with the standard evaluated. The victim validated that a caregiver handled her inappropriately.
