The caregivers responsible for the child were not aware that the child was missing from the group until the child was returned by another caregiver.

It was determined that the operation failed to report to Child Care Regulation an incident that placed a child at risk.

It was determined that a child was left unsupervised for several minutes as the children were being transitioned back inside from the playground. The child was found in the hallway by another caregiver and returned to their class.

During a DFPS investigation, it was determined that a caregiver firmly grabbed a child by the arms forcing the child to sit in a chair which resulted in bruising along the child?s upper arm. The caregiver is no longer employed.

During a DFPS investigation, it was determined that a caregiver did not interact with a child in a positive manner when the caregiver forcibly handled a child which resulted in the child being injured. The caregiver is no longer employed.

It was determined that caregivers were not aware of the children they were responsible for when a 2-year-old child was able to walk into the restroom without the caregiver?s knowledge and the child was not able to get out. Neither of the 3 caregivers realized the child had gotten into the restroom and was not present with the group. One of the caregiver?s heard the child crying and opened the door and found the child inside of the restroom.

The operation failed to report to Child Care Regulation a situation that placed a child at risk.

This standard was evaluated as part of an investigation and found to be deficient when it was determined that the director did not ensure new hire staff were properly trained on the positive physical interactions between staff and children and did not ensure that new hire staff had a full understanding of minimum standard compliance and licensing laws.

This standard was evaluated as part of an investigation and found to be deficient when the caregiver picked up children by their arms and wrist to move from one place to another.

Based on information obtained from the DFPS investigation, a caregiver was observed on video using prohibited punishment when the caregiver grabbed a child by their arm and collar of the shirt. The caregiver is no longer employed at the operation.

Based on information obtained from the DFPS investigation, a caregiver was observed on video forcing a child that was visibly upset to sit down between their legs to keep the child from moving. When the child would try to leave the caregiver, the caregiver would grab the child by the arm and/or collar of the shirt pulling the child back between their legs. The caregiver is no longer employed at the operation.

Based on information obtained from the DFPS investigation, it was observed on video that a caregiver did not demonstrate good judgment and self-control in their interactions with a child. The caregiver appeared to be frustrated with the child's behavior and handled the child in a rough manner multiple times. The caregiver is no longer employed at the operation.

The two directors and 3 caregivers will need to complete the following trainings: *Preventing and responding to emergencies due to food or an allergic reaction *Handling, storing, and disposing of hazardous materials

The two directors will need to complete transportation safety training.

Based on information obtained through a DFPS investigation, it was determined that a caregiver did not interact with a child in a positive manner as the caregiver was observed on video handling the child in an aggressive manner. A caregiver was observed on video pulling a child by their arms to the table in a hurried manner and roughly placing the child down in their chair. The caregiver was observed wiping the child?s face in a rough manner & then pushed the child's chair up to the table in an abrupt manner. As another child stood to reach for their sippy cup on the table, the same caregiver pushed the child?s chair up to the table in a hurried/abrupt manner causing the child to fall back down onto their chair.

It was determined that the operation failed to notify the parent immediately of a situation that placed their child at risk. The parent was not notified that their child was left unsupervised on the playground until the parent picked their child up from care.

It was determined that two caregivers left a child unsupervised on the playground for approximately 15 minutes when transitioning the class back inside. The operation became aware that a child was left outside when they were notified by the elementary school located next door.

It was determined after reviewing video footage that a caregiver handled two children in an inappropriate manner. The caregiver was seen on video, harshly grabbing & pulling two children up by one arm to move them. The children's feet were observed to briefly dangle in the air as they were being moved.

A caregiver demonstrated a lack of self-control and good judgment when redirecting two children. The caregiver was observed on video forcibly grabbing and pulling children by their arm which resulted in one of the children sustaining a scratch on their arm. The same caregiver was also observed on video tapping a child on top of their head with a book.

The operation did not report an incident which placed a child at risk within the required time frame.

One caregiver only completed 7 of the required 24 clock hours of training for the previous training year (2/2021 - 2/2022).

Three of the children's records were missing the telephone number for the emergency contact person that was listed.

The operation allowed a staff person to be present and have access to children prior to having the eligibility determination letter. (Note: The caregiver will not return to the operation until their eligibility status is received.)

There were 8 infants in the Infant classroom I that did not have updated feeding instructions on file. There were 2 infants in the Infant 2 classroom that did not have updated feeding instructions on file.

Five of the ten children's records evaluated did not include complete physician information.

The operation did not have an annual fire inspection scheduled or on file.

There was one electrical outlet in the infant room, in the toddlers classroom and one in another toddlers classroom. This was corrected at inspection.

Upon review of video footage and interviews, it was found that a caregiver, contracted by a substitute company, left children in a classroom unattended for over a minute. This was corrected at inspection, being that the operation discharged the employee in concern.

The operation did not report an incident where a child was injured and required medical attention within the proper 48 hour time frame. This was corrected at inspection when proper reporting procedures were discussed with the director.

An incident report was filled out but was not signed by the parent.

A caregiver used poor judgment when she forcibly grabbed a child by the arm multiple times and restrained him.

A caregiver failed to relay information regarding what a child said to her to other staff and management.

A caregiver used poor judgment when she physically restrained a child during naptime.
