14 out of 17 children immunizations were not up to date.

Two classrooms had cleaning supplies that were accessible to children. This was corrected when the caregivers moved the cleaning supplies out of reach of children.

A caregiver failed to wash an infant's hands after changing their diaper. This was corrected when the caregiver washed the infant's hands.

Two caregivers did not know how many children they were responsible for.

During snack time children were only served food from one food group which did not meet the USDA nutritional guidelines.

Information obtained throughout the investigation shows that the operation failed to demonstrate good judgement by continuously allowing children to enter a hallway closet that self locks and having to get a key to unlock the closet and let the children out.

A caregiver without a background check eligibility letter had direct access to children in care. This was corrected when the caregiver did not return to the operation from their lunch break.

Information obtained throughout the investigation shows that caregivers allowed children under the age of 2 years to participate in screen time activities.

Operation did not have a current fire inspection.

Operation did not have a current sanitation report. This was corrected during my inspection when I received receipt of payment for the inspection to be completed.

The medication authorization for a child requiring medication administration expired in April of 2024.

A caregiver was observed placing a child in the crib on their stomach to nap. Caregiver was instructed to place the child in the crib on their back for nap.

Out of ten classrooms observed, three caregivers did not know the number children they were supervising.

One classroom was using screen time with a child under the age of 2 years old. This was corrected at inspection when the tablet was turned off.

Two employees were present, going in the classroom hallway, and classrooms without completed background checks. One had an inactive status on their background check. One had a pending background check. This was corrected when both employees left .

Children from one class were observed running between rooms, standing on a table, and pulling each other by coats and blankets during inspection.

During an investigation a caregiver was observed moving two children by one of their arms, lifting them up off the floor to a new spot.

During an investigation it was found that the operation waited multiple days to notify parents of children when they were made aware that a caregiver used prohibited punishment on their children while in care placing them at risk.

During an investigation it was found that a caregiver was leading a child to another table by pushing their head along the way, pushing children?s heads to a facedown position at tables, pushing a rolling bookshelf towards a child twice, and banging a tablet on a table by children. There is evidence to suggest the caregiver restrains children by holding them tightly.

During an investigation there is enough evidence to determine a caregiver was yelling at the children in care.

During an investigation it was found that the operation did not provide an incident report to the parent within the required 48 hours.

Three chidren were lacking the required immunizations.

Six staff were lacking food allergy and hazardous material training.

A caregiver did not know how many children were in their care.

Children were observed eating lunch and watching a movie. Caregiver was informed why the television could not be on and the television was powered off.

Two caregivers were not aware of the children's ages in their care or where to obtain the information.

The caregiver did not show good judgment when she decided to carry a child in a football hold that led the child to sustaining a head injury.

One child did not have an emergency authorization on file.

Children were observed sleeping with their blankets over their heads in three separate classrooms.

One caregiver did not have training in the following areas: (1)Emergency preparedness ; (2)Preventing and controlling the spread of communicable diseases, including immunizations ; (3)Administering medication, if applicable, including compliance with ?746.3803 of this chapter (relating to What authorization must I obtain before administering a medication to a child in my care?) ; (4)Preventing and responding to emergencies due to food or an allergic reaction ; (5)Understanding building and physical premises safety, including identification and protection from hazards that can cause bodily injury such as electrical hazards, bodies of water, and vehicular traffic ; and (6)Handling, storing, and disposing of hazardous materials including compliance with ?746.3425 of this chapter (relating to Must caregivers wear gloves when handling blood or bodily fluids containing blood?).

Four staff members who transport children are missing their transportation training certificate.

One caregiver's records out of ten reviewed did not have up to date CPR & first aid certification.

The operation did not have a CPSC form. This was corrected at inspection when the director was provided a form and it was signed.

The operation's vehicle safety alarms within their 4 vehicles used did not operate properly.
