The operation has not updated/verified liability insurance information by the annual due date. Verification was due by 10/24/2025.

Based on the investigation findings, this standard was reviewed and found to be in non-compliance with The Minimum Standards for Child Care Centers. The operation transported two three-year-old children; the operation only had one forward facing child safety seat on the vehicle available for use. The operation staff did not know the weights or heights of any the fifteen children ages 4 through 7 years old that were transported; the operation could not determine if these children required more than a safety belt.

Based on the investigation findings, this standard was reviewed and found to be in non-compliance with The Minimum Standards for Child Care Centers. On 06/09/2025 the operation went on a field trip to a water park where there were twenty-four children with three staff members in the wading pool areas. The youngest children on the field trip were three years olds; the operation was over ratio by six children.

The infant classroom was observed to be out of ratio. Their were six infants with one caregiver. The specified age group was 12 to 17 months, the classroom was out of ratio by one child. Durinig the inspection a second caregiver stepped into the classroom to bring the class into ratio/group size.

The total number of employees who ceased working at your center in 2023 has not been reported as of 1/16/2024.

The AC unit on the smaller playground need a barrier that children can not have access to.

It was disccused with the person director and person in charge that all of the broken bicyles need to be fix or remove them out of children's reach.

In reviewing 6 staff files: - Two were lacking TB test results

One student is still lacking a food allergy action plan.

The operation s emergency preparedness plan was lacking: #1b, 1c, 2a, 2b, 3b, 4 and 5.

A nap room with 19 children was left unsupervised during today's inspection. The caregiver had left the room to take the 20th children to another room.

Two caregivers have not completed high school and do not have high school diplomas on file.

The two students with food allergies were lacking the food allergy action plans.

The infant room was observed with 7 infants, ages 6m-11m, with only one caregiver.

The prek room was observed with 11 children and no caregiver during naptime.

In reviewing 6 staff files: - Two were lacking education qualifications - Two were lacking TB test results - Two were lacking Employment affidavits - Two were lacking documentation showing they received the personnel and operational handbooks. - Two were lacking documentation showing preservice and orientation was complete.

The operation did not have a posted list of allergies.

The caregiver in charge of the toddler room was observed texting while supervising children.

The operation has not completed F2885 for the CPSC recalls since April 2018. This is to be done annually.

Four caregivers did not have all the required 24 hours of annual training for last year or this year. The 5th caregiver has worked at the operation less than a month. This included lacking all the required topics.

An 11m old infant was observed sleeping in the bouncer.

Neither driver had documentation of current transportation training.

The director had only 15 of the required 30 hours of annual training for last year and only 2.5 of the 30 for this year. This included lacking all the required topics.
