A witness observed an employee disciplining their own child utilizing prohibited punishment while the child was in the care of the operation.

Staff did not practice good judgement when they addressed issues amongst each other in a classroom with children present. Voices became elevated during this incident.

There have been multiple incidents in which staff have stepped out of their classroom leaving children unsupervised during naptime.

Two caregivers did not wash their hands after removing gloves after changing a diaper. This was corrected at inspection when it was addressed with the caregivers.

4 out of 4 employee records were missing the pre-employment affidavit.

3 out of 5 classrooms observed had out of date activity plans posted.

In the toddler room, diapers were observed in an uncovered container on the floor. In the Pre-K classroom wipies and diapers were observed stacked on a chair. In both classrooms the items were in reach of the children.

Several children were observed eating crackers and cheese sticks on a bare table.
