The infant Classroom was out of ratio; one caregiver was supervising 5 infants with the specified age of the children in the classroom being 0 - 11 Months old. NOTE: This was corrected at inspection when a caregiver stepped into the room making the classroom in ratio.

The time and dosage of a medication given to a child is not being written when medication is given to child.

Thirteen of the children being transported are missing emergency medical transport and medical authorization forms. Corrected during inspection.

Two caregiver records of the eight records reviewed are pending required content for annual trainings. Caregivers are missing: Caregiver 1- Medication Administration, Communicable Diseases, Allergic Reactions Caregiver 2- Communicable Diseases

In the outdoor area there was a large hole near the fence making it a hazard.

A diaper cream was seen in the infant room that was not labeled with a child's name. NOTE: This was corrected at inspection when caregiver labeled the diaper cream with the correct child's name.

During inspection it was observed that in one of the classrooms the emergency light was not working. Note: This was corrected at inspection when director placed a working flashlight to replace the flashlight that was not working.

Surface of the diapering mat in the toddler is torn on both sides.

Vehicle used during today's transportation route did not have operation's name/telephone numbers, children's emergency medical/information forms and fire extinguisher. Note: This was corrected during inspection when provider transfered these items from the older van into the vehicle currently being used for transportation.

There is chipping paint on the walls of the prek classroom.

15 passenger van uses during today's transportation route does not have an electronic child safety alarm.

Operation does not have an emergency plan for a child with a diagnosed food allergy.
