Staff are missing the following training topics; -Four staff are missing preventing and controlling the spread of communicable diseases, -Three staff are missing preventing and responding to emergencies due to food or an allergic reaction; -Six staff are missing handling, storing, and disposing of hazardous materials.

The governing body has not ensured that the minimum standards are being followed at the center and corrections have not been made.

A caregiver did not use appropriate verbiage when discussing a child's behavior in the presence of the child.

It was determined that a parent was not notified by pick up time that their child had been bitten. The staff were unaware that the child had gotten bitten.

The school age playground has a broken platform. The border on the school age playground has the metal stakes coming out. The outside wooden fence is breaking in multiple spots and has nails sticking out. The older school age classroom has a broken outlet cover. The four year old classroom has a broken base board under the sink.

1 board is need of replacement at the northwest corner of the playground. 2 weeks will be given to address this issue.

The operation does not have a current fire inspection.

This deficiency was corrected at inspection when the director instructed a caregiver to move the correct level of wood chips in front of the slides use zones. The operation has ordered additional wood chips to keep the playground's loose fill properly maintained.

A caregiver in room one did not wash an infant's hands after two diaper changes.

Loose fill is not being maintained at the required level of six inches.

The current director has been in place since 12/27/2022 and has not provided director credentials to Licensing. The governing body did not inform Licensing until 02/10/2023 that a new director had been put in place.

The fencing around the playground has come separated from the top creating a safety risk to children in care. The trim on bottom of one of the classrooms is detached and in need of repair.

This was evaluated during the investigation and determined to be deficient. Operation was notified medical treatment was required on Friday 12/02/2022 and reported to CCR the following Tuesday 12/06/2022 going past the required two-day time frame.
