One child's file did not have the current immunization record.

Ceiling tiles need replacement due to past water leaks. Wall in restroom has a hole and needs repair. Wall and ceiling vents had build up of dust.

Exposed pipe in classroom needs to be covered.

Playground areas are below the standard proper depth for loose fill.

Two play structures have rust and peeling paint. One of the play structures has two rusted screws that are a hazard. Wooden board on playgrounds are broken and leaning to the side.

Two immunization records were not current.

A child returned to the playground without the staff's knowledge. Caregivers immediately retrieved the child after realizing the child was not in the classroom.

At the time of the inspection 2 of the 10 records reviewed showed children with food allergies, but without any documentation from health care professionals regarding a plan for if the child has an allergic reaction.

At the time of the inspection in the toddler play area there was a gap under the fence estimated to be 4 inches. This caused concern for the gap being large enough to be an entrapment hazard to the age group present. In the large play area a panel of the wood fence was observed to be loose and only connected by 1 screw that was not drilled in completely. There is concern that older children could easily pull the panel off, exposing screws and creating an opening in the fence. The fencing near the secondary exit to the playground the fencing is not completely connected to the posts. When pushed this fencing rocks away from the posts and there is concern for it falling over.

This standard was evaluated in regards to a DFPS investigation and was found to be deficient. Information from the investigation showed that 3 caregivers had concerns of a caregiver acting rough with children, but none of the caregivers reported these concerns to the director or Licensing.

A prescribed epi pen on site expired on 24 Sep 2021. A photo was taken. Medication will be returned to parent at pick up today.

A food allergy emergency plan was not on file for a child with a known food allergy.

The caregiver did not demonstrate self-control when she reacted to a child's behavior by pinching him.

A caregiver admitted that she pinched a child while disciplining a child in care.

The toddler room was over ratio by 8 children.The ratio was 17:1. The specified age group was 18-23 months, which only allows for 9 children.

During naptime, the toddler room reduced ratio with 5 children under 18 months. The ratio was 17:1. SAG: 18-23

The infant room was over ratio by 1 child. The specified age group was (0-11 mo). There were 11 children in the classroom.

The director did not demonstrate good judgment by failing to notify both Child Care Regulation and parents of non-working toilets and classroom flooding. Good judgment was also not demonstrated by allowing the continual use of classroom toilets that could not be flushed for several hours.

Children were allowed to continue to use non-flushing toilets for several hours. Children were not able to properly wash their hands after using the bathroom due to the water being turned off for repairs.

Parents were not notified of an unsanitary occurrence at the daycare.

The director failed to report an unsanitary occurrence to Child Care Regulation.
