A 3 year old child was left unsupervised in the restroom. CORRECTED: Director approached the restroom area to assist child.

A health statement was not submitted within the 12 months of enrolement date.

Parental notice of lack of liability insuranced form was not avilable for review.

Four out of four children's records reviewed are missing the updated special care statement. One file is missing health statement.

Lesson plan for the week was not available for review nor posted. Infant daily schedule does reflect an AM opportunity for outdoor play.

Operational policies were reviewed and the following information is missing: (1) Hours / Days of Operation (18)(f) Outdoor Activity - Extreme weather criteria (30) Procedures for supporting inclusive services to children with special care needs.

A caregiver was providing care for 6 children ages 13mo-22mo, SAG 12-17mo. Classroom was over ratio by one child. Note: during inspection one of the children was transfered to another classroom placing room back within ratio.

Trash can by the diaper changing station does not have a lid and was observed with used diapers. Note: This was corrected at inspection when trash can was emptied and trash can was replaced with one with a lid.

One caregiver was caring for four children ages 3mo to 4yo in the multipurpose room. One caregiver was caring for 8 children ages 16mo to 3yo in the prek room.

There are cleaning products accessible to children in the cabinet underneath the boy's sink.

Food Allergy information for two children is not posted in the kitchen and information for one child is not posted in the child's classroom.

Changing mat in the infant room is torn.

One of the infants room was changed into a toddler classroom. Note: This was corrected at inspection when operation provided an updated floor plan and agreed to update infant's capacity.

Last fire drill conducted on 02/08/2023 was completed in four minutes.

Two out of four staff files reviewed are missing four hours of pre-service training.

Toddler-age children were not served a vegetable portion during lunch and Infant-age children were not fed a meat (or alternative) protein portion.

One out of four children's records reviewed is missing a food allergy plan.

One qualified caregiver and an employee of the operation were caring for 6 children ages 6mo-12mo, SAG 0-11momo. Classroom was over ratio by two children.

Menu posted did not match items served during lunch.

Lesson plan posted in the prek room did not match planned activities for the week and was dated June Week 1. This week's written lesson plans for infant-age children was not posted.

There was a plastic tub with water in the playground. There were two disinfecting spray cans in the girl's restroom, one by the sink and the second underneath the sink. There were two hand sanitizer bottles and a disinfecting spray can underneath the sink of the infant's room. The cabinets were not locked. Note: This was corrected when cleaning products were removed and/or placed in locked cabinet and water from plastic tub was dumped.

Operation does not have a carbon monoxide detector.

Operation does not have a smoke detecting system.

Emergency Preparedness Plan was reviewed and missing the following: -Staff responsibilities -how will children with disabilities and under 2yo will be evacuated -how will attendance will be checked, -how will operation communicate with local authorities -how will staff evacuate and relocated essential information -how will staff care for children until they are released -how will children be reunify with parents

Play-yards in the infant room are not labeled with the children's name or assigned number. This was correct at inspection when provider labeled with play-yards with the children's names.

Three infant bottles observed during inspection were not labeled with the children's name. This was corrected at inspection when provider labeled the bottles.

Two out of five children's files reviewed were not available at the time of inspection.

One of our two staff files reviewed is missing a notarized affidavit. A second files is missing copy of high school diploma or ged, notarized affidavit, orientation statement and 8hr of pre-service training.

School-age classroom did not have interest centers set up during inspection. NOTE: This was corrected during inspection as interest centers were set up and new material was added.

Emergency lighting was not present inside 6 out of 6 classrooms at the operation. NOTE: This was corrected during inspection as battery-powered lighting was added to each classroom.
