The transportation van did not have the required safety alarm.

The fire extinguisher inside of the vehicle was not approved by a state or fire marshal.

The two after school playgrounds lacked the proper nine inches of loose-fill surfacing around the play structure.

Inspector observed one person present at the operation who was allowed unsupervised access to children and did not have an initial background check or FBI fingerprint submitted and on file.

Inspector observed one caregiver step out of the room to get a stroller for the infants to go outside leaving the infants unsupervised.

Three out of four staff members were missing trainings in section (e) of this standard. One staff member was missing training in food allergies and was missing 1 out of 24 hours. One staff was missing training in food allergies, building and premises safety, hazardous materials, and emergency preparedness. One staff was missing training in SIDS, shaken baby, and early brain development, communicable diseases including immunizations, administering medication, food allergies, building and premises safety, hazardous materials including blood borne pathogens, and emergency preparedness. This staff member had 7 out of 24 hours on file.
