A caregiver did not use good judgment when addressing a child for biting, the caregiver would tap the child's mouth and state no-bi-ting. After each syllable the caregiver would tap the child's mouth.

A caregiver without a cleared background check was present caring for children at the center.

A caregiver disciplined a child by using negative comments.

Operation did not correct the fire inspection deficiencies.

During the inspection there were 3 out of the 5 classrooms that had the TV's on with non educational videos, the videos did not go with the lesson plan. NOTE: This was corrected at inspection when caregivers turned off the TV's

Caregiver did not wash hands of the child after diaper change.

Based on the evidence gathered during the investigation, a caregiver exercised poor judgment by using her foot to keep a child from rising off the mat during naptime.

Fire drill was not practiced on February 2024. Last fire drill was practiced on January 10, 2024. Drill was practiced during inspection.

One caregiver was supervising 15 children with a specified age of 2 years old. They were out of ratio by 4 children. Ratio is suppose to be one caregiver for 11 children with a specified age of 2 years old. This was corrected during inspection by adding another caregiver.

One of the five employee records reviewed was missing emergency preparedness, preventing the spread of communicable diseases, administering medication, allergic reactions, hazardous materials, and building safety from annual trainings.

Reviewed transportation binder and the medical information form for all the children being transported was missing.

Fire inspection expired on 8/8/2023.

Three electrical fans were observed not being anchored and at the reach of the children in the infant and toddler rooms. Note: The three electrical fans were removed by caregiver during inspection.

The children in the toddler room the children were watching a Christmas program on the TV. NOTE: This was corrected at the inspection when the caregiver turned off the TV.

In the infant room there were two cribs with bottles and blankets making it a hazard for the infants. NOTE: This was corrected at inspection when caregiver removed the blankets and bottles from both cribs.
