This rule is being evaluated as part of an investigation and found to be incompliance. Child was left unattended in a classroom and was found by another caregiver in the building.

A caregiver was found sleeping in a classroom while children are in her care.

Based on the preponderance of evidence of the investigation by DFPS, a caregiver observed leaving the room on 3 different occasions where she has no visual and/or auditorial of the children.

Based on the preponderance of evidence of the investigation by DFPS, a caregiver has grabbed a child by upper arms and takes him out of one chair and moves him to another chair.

Based on the preponderance of evidence of the investigation, and observation by the Childcare Investigator from DFPS, a two year olds child was observed in a toilet for almost two hours.

Based on the preponderance of evidence of the investigation by DFPS, a caregiver observer flicked a child nose.

This standard was found deficient as part of a DFPS investigation. A caregiver admitted to leaving children sitting on a potty chair for 30 minutes at a time, while potty training.

This standard was evaluated at the result of the DFPS investigation, and the standard was found to be deficient as the caregiver was found reason to believe for physical abuse. The caregiver was found flick children on the nose and mouth. The caregiver was swatting children on the bottom and slapping them on the cheek. There was bruising to a child thigh where the caregiver was observed to have grabbed the child earlier in the day. Also, as the result of DFPS investigation and the standard was found to be deficient as the second caregiver was found reason to believe for neglect supervision. The second caregiver in the classroom observed the behavior and prohibited discipline by the other caregiver in the room and did not intervene or report it to management.

This standard was found deficient as part of a DFPS investigation. A caregiver was observed on video to flic a child on the mouth and nose and slap a child in the face.

This standard was found deficient as part of a DFPS investigation. The caregiver did not demonstrate good judgment when using excessive force and inappropriate discipline children. The second caregiver did not demonstrate good judgment by not intervene and report to management as she was present in the classroom while her co-worker uses excessive force and inappropriately discipline children.

This standard was found deficient as part of a DFPS investigation. A caregiver was observed on video to forcefully grab a child and place the child forcefully in a chair.

An infant fell asleep during floor time. The caregiver stated that the child fell asleep 5 minutes before I observed the classroom. I walked back to the room 10 minutes later and the child was still sleeping on the floor.

The facility did not have the inspection from from the previous inspection posted on the parent board.

The caregiver who attended to the injured child did not have current first aid training. Her certificate expired in 3/2023.

The scissors being used in the 2yr old class are specified for use by kindergarden and first grade.

The caregivers' training files were not available for review upon request during the 04/27/2023 inspection.

A group of 11 children were observed without an activity for 23 minutes. This occurred after two additional caregivers joined the classroom with 4 toddlers.

The caregivers did not know who was responsible for the child, each of the three caregivers stated that they were not responsible for the child when the incident occurred.

Three caregivers did not demonstrate good judgment when they had a 10 minute long conversation amongst themselves while caring for children. Two caregivers did not demonstrate good judgment when they observed three children rolling around on each other on the carpet and did not intervene. This incident occurred for 2 minutes.

A caregiver was observed seated not free from administrative duties for 13 minutes while caring for children.

The use zone surrounding the slide on the Pre-K playground was observed with less than an inch of wood chips. A picture was taken at the time of the inspection. Grass was observed growing in the use zone of the rock climbing wall on the Pre-K causing the wood chips to become compacted. Grass was also observed growing on the sand hill that acts as loose fill for the entrance of the slide.

The operation notified the health department but did notifiy licensing of a person who tested positive for COVID-19.

The operation failed to inspect the playground before children use.

This standard was under review due to an investigation and was found deficient. The caregiver did not show good judgement when she took the child to the bathroom area, to talk to a child, about his behavior.
