Parents rights were not included in the parent handbook.

In the front outside play area there are potential hazards such as loose bricks, a damage fence on the side of the house, the two entrances gates leading to the street are not secure and are accessible to children. The staircase located on the side of the porch needs to be reattached to the porch to prevent injury to a child. Additionally, a long nail and sharp pieces of wood were observed in the backyard play area.

Documentation for safety drills, fire extinguishers, smoke and carbon detectors were not made available.

In the front outside play area, a pile of loose wooden boards with nails exposed were made easily accessible to children in care. Additionally, loose bricks were made accessible in the front play area potentially creating trip hazards for the children.

In the front bathroom, shampoo, conditioner, mouthwash, and other toxic household items were made accessible to children.

Upon arrival, the children and primary caregiver were in the front outdoor play area where one child was found hiding in the corner standing on a chair behind a crib, another child was seen walking towards the backyard while the caregiver was near the front of the house. During the walkthrough, one child ran towards the back of the home while the primary caregiver was still at the front door letting the other children inside. The 2 year old was seen in the bathroom in the back standing on a toddler tower step stool leaned over the sink in the dark by themselves. Two toddlers were observed by the front door without the caregiver present or nearby. Various times toddlers were seen running through the home without supervision.

There is no posting for a child in care with a diagnosed food allergy.

When reviewing records, it was found that a child in care has a diagnosed food allergy with no food allergy emergency plan on file.

In the operation's handbook there are no instructions on how a parent may contact the local Licensing office, access the Texas Abuse and Neglect Hotline, and access the HHSC website.

There was no documentation for testing the fire alarm for the months of June and July.

There was no documentation of fire extinguisher inspections for the months of June and July.

There was no documentation of testing the carbon monoxide detector for the months of June and July.
