A child in care did not have proof of the parent receiving information regarding the safe sleep for infants policy, the parent's rights, and the animals in the home.

The last three months of menus were not available to be reviewed as the September month was not available upon request. The last three months of sign in and out logs was not avaiable for review as the September month was not available for review upon request.

The parent's rights form was reviewed. The operational policies regarding information to parents about the pets/animals in the home was recited as it has not been completed.

There were eight children present at inspection. There was one child that was birth to 17 months of age and a total of 7 children that were 18 months to 5 years old when the max number of children that can be present that are 18 months to five years of age is 5 total children. Therefore, the provider was over by two children for the daily ratio regardless of there being more than one caregiver present due to the permit type issued to the operation.

A child that is in and the parent had noted on the admission form that the child has a food allergy did not have the food allergy in file for review.

The permit holder did was lacking 5 hours of training within the most complete training year of 1/28/2024 - 1/28/2025.

The two-hour transportation training was not available for review for three adults that provide transportation to children in care.

A household member that resides in the home and is older than 14 years of age has been doing so without an eligible notification from the background check unit.

A child file reviewed did not have any immunizations in file for a child that is in care. **This was corrected at inspection when the parent provided an immunization record for the child to the operation.

The first aid kit was missing a thermometer and scissors. **These were provided by the provider and placed in the first aid kit in the vehicle.

The transportation training was not available for two caregivers that provide transportation training to children in care.

The operation has not provided the Parent's Rights information to parents as required of the operational policies and did not include a policy with iinformation to parents about the promotion of physical activities. Additionally, updates are required for the policy to pets/animals in the home.

A small plastic pool was observed not stored out of children's reach. Additionally, there was a dead chicken next to the pool in a play yard that had been used by children previously.
